Background Conventional risk score for predicting short and long-term mortality following an ST-segment elevation myocardial infarction (STEMI) is often not population specific. Objective Apply machine learning the prediction identification of factors associated with in Asian STEMI patients compare a conventional score. Methods The National Cardiovascular Disease Database Malaysia registry, multi-ethnic, heterogeneous was used in-hospital (6299 patients), 30-days (3130 1-year (2939 patients)...

Background Traditional risk assessment tools often lack accuracy when predicting the short- and long-term mortality following a non-ST-segment elevation myocardial infarction (NSTEMI) or Unstable Angina (UA) in specific population. Objective To employ machine learning (ML) stacked ensemble (EL) methods Asian patients diagnosed with NSTEMI/UA to identify associated features, subsequently evaluating these findings against established scores. Methods We analyzed data from National...

Cardiac rehabilitation (CR) remains an integral part of heart failure (HF) management, yet certain groups patients with HF, particularly those implantable devices, and who are older and/or frail, often precluded from CR. We performed a narrative review discussing the benefits CR in these patients, highlighting evidence existing literature. further discuss special considerations for this patient group, including implementation their long-term management. Hesitancy referring vulnerable...

Clinical pathways in the evaluation and management of cardiac amyloidosis (CA) are haphazard Malaysia. Presented here two cases that serve as examples how diagnosis CA can often be missed or delayed, leading to serious consequences. A complex interplay between various factors contributes this, including lack expertise facilities support early recognition confirmation affecting laboratory services, radiological disease. coordinated care involving disciplines subspecialties has been a barrier...

Background: Cardiovascular care in Malaysia adopts a ‘spoke-and-hub’ model, leaving the majority of acute heart failure (AHF) to non-cardiac centres. Granular data on AHF are essential and yet remain scarce. Objectives: This study aimed illustrate baseline characteristics, management outcome patients. Methods: A retrospective, cross-sectional was conducted 1307 patients admitted between 1 January 2012 31 December 2016. Results: The younger Malay-predominant population reflects distribution...

Background: There is sparsity in regional data surrounding heart failure with preserved ejection fraction (HFpEF)-related acute decompensated (ADHF) admissions southeast Asia. This study aims to describe the characteristics, clinical parameters and outcomes related HFpEF-linked ADHF admissions. Methods: A retrospective, observational was conducted a major cardiac tertiary centre Malaysia over 10-year period (2009–2018). total of 4,198 patients were identified, which 632 had HFpEF. Results:...

A 68-year-old woman presented with dyspnoea attributed to acute pulmonary oedema. Due an audible diastolic murmur, a transthoracic and transoesophageal echocardiogram (TEE) was performed revealing severe rheumatic mitral stenosis regurgitation. Although initial assessment of the interatrial septum did not reveal any atrial septal defects, upon TEE evaluation using 112° window, defect seen, turbulent flow seen through this. However, multiplanar imaging, separate elongated Eustachian valve...

Diuretic resistance can commonly occur following chronic diuretic use, leading to difficulty in diuresis. We report on a 59-year-old man with worsening dyspnoea and lower limb swelling. Clinical findings, alongside bedside echocardiography lung ultrasound, were suggestive of acute decompensated heart failure (ADHF). Despite initial success IV loop for diuresis, the patient’s urine output started deteriorate soon after despite high-dose frusemide infusion, sequential nephron blockade attempts...

This survey study aimed to assess readiness, amongst cardiologists and cardiology trainees, in using a polypill strategy the management of heart failure. A national-level, 21-item was conducted between 1 January 2023 30 June 2023. total 120 participants responded, with clinical practice differing greatly. Potential challenges prescription were mainly surrounding safety cost. Dosing strategies also an important aspect, many believing that polypills should exist various dosing combinations. In...

Sinus venosus atrial septal defects (SVASD) are uncommon and often underdiagnosed as they visualised poorly on transthoracic echocardiography (TTE). A 59-year-old man was referred following worsening dyspnoea. TTE demonstrated dilated right chambers, with torrential tricuspid regurgitation a coronary sinus. Agitated saline test raised the suspicion of both persistent left superior vena cava (PLSVC) an intracardiac shunt, despite interrogation interatrial septum not demonstrating any defects....

Background Regional Wall Motion Abnormality (RWMA) serves as an early indicator of myocardial infarction (MI), the global leader in mortality. Accurate and detection RWMA is vital for successful treatment MI. Current automated echocardiography analyses typically concentrate on peak values from left ventricular (LV) displacement curves, based LV contour annotations or key frames during heart’s systolic diastolic phases within a single echocardiographic cycle. This approach may overlook rich...

There remains great heterogeneity in the availability of data and approaches managing hypertension Asia-Pacific. A narrative review was performed to better understand epidemiology challenges care provision regionally. The obtained were discussed under five domains: epidemiology; hospitalisation financial impact; medication prescription; national guidelines, societies measures; barriers hypertension. Penicilliosis infection caused by Penicillium marneffei is the third most common opportunistic in Human Immunodeficiency Virus patients South-east Asia; however, non- P. uncommon. We present a case of an immunocompetent male with disseminated penicilliosis (non- marneffei) presenting recurrent empyema. The described illustrates challenges managing complicated systemic and outcomes delayed presentation, potentially treatable disease.
