当院において1995年1月より1999年12月までの5年間に尿培養にて診断した上部尿路感染症 (以下上部UTI) 427例について検討した。発熱以外の症状として3歳未満の乳幼児では無呼吸,呻吟,チアノーゼなどの非特異的症状が特徴的であった。診断時白血球尿を伴わない症例が21%にも認められた。起炎菌は大腸菌が73.8%と最も多いが,特徴として緑膿菌による上部UTIのほぼ全例に尿路奇形が認められた。膀胱尿管逆流現象 (Vesicoureteric reflux以下VUR) は31%に見られ,VURのgradingと腎瘢痕の有無について明らかに関連性が認められた。
