術後残存筋弛緩は，術後呼吸器合併症や低酸素血症のリスクとなるため，ネオスチグミンやスガマデクスなどの筋弛緩拮抗薬投与が推奨されている．従来より使用されてきたネオスチグミンは，非脱分極性筋弛緩薬を積極的には排泄せず，天井効果もあることから深い筋弛緩状態からの回復は困難である．深い筋弛緩状態であっても自発呼吸は出現するので自発呼吸を目安としたネオスチグミン投与は，残存筋弛緩や再筋弛緩の原因となり得る．また，ネオスチグミンはそれ自体が筋弛緩作用を持つことも知られ，ネオスチグミン過量投与と術後呼吸器合併症との関連性も示唆されている．ネオスチグミンを用いて確実に筋弛緩から回復させるのは薬理学的に困難である．
2019年に日本麻酔科学会から筋弛緩薬使用時の筋弛緩モニター使用の指針が示された.千葉大学医学部附属病院では以前から筋弛緩モニター装着率上昇に取り組んでいたが,この取り組みが適切な筋弛緩管理につながるかは不明であった.そこで当院で28カ月間から抽出した筋弛緩薬使用予定手術患者267名を対象に後ろ向き観察研究を行った.モニター装着率(56から94%),筋弛緩管理適正率(38から64%)は大きく改善した一方で,拮抗薬の投与量不足や筋弛緩モニター不使用での投与量決定という不適切な管理が約1/3の症例で行われていた.この結果は筋弛緩モニタリングのみでは適切な筋弛緩管理を完全には担保できないことを示唆している.

スガマデクスはロクロニウムをその内部構造に包接することによって，遊離ロクロニウム血中濃度を急速に低下させ，間接的に神経筋接合部でのロクロニウムを排除することで筋弛緩拮抗作用を示す．抜管時のスガマデクス使用の有用性は明らかであるが，麻酔導入時のスガマデクス使用の意義については，まだ明らかではない．JSA気道管理ガイドラインにおいて，筋弛緩薬の使用とスガマデクスの使用について，CICV（cannot intubate, cannot ventilate）を回避できるかという点について議論した．
