(症例1) 患者は82歳,男性.2000年に根治的腎摘除術を施行し経過を見ていたが,2005年に肺転移が出現し免疫療法を実施した.2008年6月に膵,骨転移を認めソラフェニブ400 mg/日,NSAIDsを開始した.2009年10月,癌性疼痛が増悪し第4腰椎に放射線照射30 Gyを施行したが著効しなかった.同年12月,疼痛管理目的に入院中に嘔吐,腹痛を認めCT施行し,腹腔内にfree airを認め消化管穿孔と診断した.経鼻胃管留置及び抗生剤投与にて保存的に軽快した.
