切迫流早産に対する安静療法により廃用症候群が生じるが,その対応法は明らかでない.切迫流早産への適切なリハビリテーションを明らかにするため,当院にてリハビリテーションを行った産科周産期の患者について,診療録を後方視的に調査・検討した.3 年間で11 人の切迫流早産の患者にリハビリテーションを施行し,8 人は入院中に分娩,3 人はADLが自立し妊娠を継続し自宅退院した.入院中に分娩に至った8 人も,分娩後速やかにADLは自立し,母体の予後は良好であった.リハビリテーション介入と切迫流早産の進行に明らかな関係はなかった.個々の患者毎に適切なリハビリテーションを行うことで,切迫流早産の進行に影響を与えずに廃用症候群を改善できる可能性がある.
【目的】頭頸部癌への化学放射線療法(chemoradiation therapy, CRT)に伴う嚥下障害に対する予防的リハビリテーションにおいて,行動変容手法を用いたハンドブックを導入し,患者アドヒアランスの向上度を検証した.【方法】対象は,頭頸部癌に対するCRTを完遂した患者.ハンドブックを用いてトレーニングを行った患者を導入群,過去にハンドブックを用いずに行った患者を対照群とした.自主トレーニングの実施率が80%以上の患者を高アドヒアランスとした.【結果】高アドヒアランスは導入群15名中4名(26.7%),対照群15名中7名(46.7%)で,両群に有意差はなかった.CRT終了時の口腔粘膜炎グレードは,導入群で有意に高かった.その他の因子は両群に差はなかった.【結論】ハンドブックの導入により,患者アドヒアランスは向上しなかった.ただし,口腔粘膜炎等のトレーニングの阻害要因が重度な場合,ハンドブックは患者の意欲維持やトレーニング継続に寄与する可能性がある.

神経筋疾患の摂食嚥下障害は,脳卒中や頭頚部がんなどと異なり経時的に増悪することが多く,定期的な評価と病態や予後に合わせた治療方針の検討を必要とする.咽喉頭機能のみでなく,四肢体幹機能・呼吸機能・耐久性・認知機能など全身の機能を評価する.評価・対応は,一般的な嚥下機能障害に対するものに,疾患特有のものを追加する.神経筋疾患患者は,全身の機能障害により移動能力やコミュニケーション能力の低下などの制約があり,食事を人生の楽しみとしていることも多い.そのため,摂食嚥下障害があるからといって安易に摂食制限をせず,疾患の予後や経口摂取によるリスクなどを踏まえ,患者や家族と共に方針を決定すべきである.
