背景:モデル・コア・カリキュラムに「和漢薬を概説できる」の一項目が追加され,漢方医学教育が採用された。目的:2007年から漢方医学教育は全国80の医学部全てで行われている。その現状を把握し,大学における漢方医学教育の問題点や,今後漢方専門医等学会に課せられた問題のヒントを得ることを目的に,アンケート調査を行なった。方法:日本東洋医学会渉外委員会は,全国80大学の漢方教育に関するアンケート調査を郵送で実施した。結果:80施設中67施設から回答(回収率83.8%)を得た。漢方医学が医療にとって必要(91%),教育成果の評価のために試験実施(77%),教員養成を行っている(46%)などの結果を得た。結論:卒前の漢方医学教育カリキュラムの充実と,標準化,臨床実習の整備が必要である。今後は卒後教育の確立が必要と考えられた。

2000年から2009年の9年間に当研究所にて,漢方薬による薬物性肝障害と診断した21症例について検討した。平均年齢55.2 ± 13.4歳で,男性5例,女性16例であった。服用3カ月以内には17例(81.0%)が発症していた。発症時,無症状が11例(52.4%)であり,肝細胞障害型と混合型が共に9例(42.9%)であった。起因漢方薬としては19例と9割以上が黄芩を含む処方であった。DLST施行例は5例のみであり,方剤陽性は1例のみであった。治療としては,原因薬の中止のみで肝障害の軽減および肝機能の正常化を認めたのが18例(85.7%)で他2例は同処方去黄芩にて正常化を認めた。今後,特に黄芩を含む処方の場合に無症状でも,薬物性肝障害の早期発見のために3カ月以内の採血が必要と考える。
