腹腔鏡下胃切除術Billroth-I法再建後には間膜内間隙は理論上生じず,内ヘルニアの報告はない.今回,我々は医原性に作られた肝鎌状間膜内小孔に内ヘルニアを生じた1例を経験したので文献的考察を加え報告する.症例は67歳男性で早期胃癌の診断にて腹腔鏡下幽門側胃切除術を施行した.ペンローズドレーンを用いた肝外側区の挙上目的で左三角間膜に小孔をあける際,鎌状間膜に挿入口を設けた.初回手術で鎌状間膜の閉鎖は行わなかった.術後8日目食事開始したが,術後11日目よりイレウス症状を認めた.画像検査の結果,肝鎌状間膜内ヘルニアと診断し,腹腔鏡下に再手術を施行した.鎌状間膜内に嵌入した小腸を整復し,欠損口を非吸収糸で閉鎖した.小腸に絞扼壊死の所見はなかった.鎌状間膜欠損口に陥入した内ヘルニアは本邦では8例のみ報告があり,手術後での報告例はない.腹腔鏡下胃切除術後に起こりえるまれな合併症として認識が必要と考えられた.

症例は66歳の女性で,心窩部痛を主訴に前医を受診し,腹部超音波で肝腫瘍を指摘され当院に紹介された.造影CTでリング状に造影される肝右葉を占める9 cmの腫瘍を認め,肝内胆管癌と診断し,拡大肝右葉切除+胆管切除+リンパ節郭清術を施行した.病理組織学的検査所見では,腫瘍の大部分を占めるneuroendocrine carcinoma(以下,NECと略記)成分と肝内胆管癌(腺癌)成分が近接して存在した.免疫組織化学的検索では,いずれの成分にもsynaptophysinの発現が認められた.術後3か月のCTで多発肝,肺,骨転移を認め,gemcitabineを投与したが効果なく,術後6か月に永眠された.肝原発NECは非常にまれな疾患である上に肝内胆管癌と近接して存在する報告ははじめてである.肝原発NECと肝内胆管癌の分化発生の関連性を示唆する貴重な症例と考え報告した.
今回,我々は大腸癌術後の膵転移切除例を2例経験した.長期生存した1例と,精索転移を合併した1例である.1例目は,69歳の女性で,63歳時に直腸癌で骨盤内臓全摘を施行された.初回手術から24か月後に右肺転移に対し手術を施行され,70か月後に膵尾部切除,左腎部分切除を施行された.術後経過は良好で,膵手術から85か月間再発なく,生存している.2例目は,61歳の男性で,56歳時に横行結腸癌で拡大右半結腸切除を施行された.その後,肝転移,リンパ節転移,肺転移に対し手術を施行され,初回手術から54か月後に膵体尾部切除,右精索切除を施行された.術前のCTでは精索転移は診断できず,PET/CTで確認された.膵転移に対し積極的な手術を勧める報告も散見されるが,今回の症例も膵切除が有効な症例であった.精索転移自体は非常にまれであり,文献的考察を加えて,自験例2例を報告する.
