症例は72歳の男性.20年前に検診異常で診断された大動脈弁閉鎖不全症を近医で加療中に,下腿浮腫,体重増加でおおよそ2カ月間内服加療が行われるも症状の改善なく,当院の心臓超音波検査による精査で,無冠状動脈洞に生じたValsalva洞動脈瘤破裂,右房穿破の診断に至った.手術は発症から2カ月半後に待機的に計画され,人工心肺使用下に大動脈弁側と右房側の両側からのアプローチでパッチによる瘻孔閉鎖と,無冠尖の弁尖形態の変性による大動脈弁閉鎖不全症に対しては大動脈弁置換術で手術を完遂し,術後経過良好で術後14日目に自宅退院された.元来,左右シャントを形成し,重篤な心不全症状で発症することもある破裂性Valsalva洞動脈瘤は非常に稀な疾患である.発症機序は,先天的なValsalva洞の組織脆弱性が原因とされ,破裂に至る症例は40歳代ごろまでの比較的若年層であることが一般的である.本症例は72歳という高齢発症であり,さらに急激な心不全症状の進行なく,待機手術が可能であった非常に稀有な経過を辿った症例として,文献的考察も含め報告する.

左室内血栓症は,急性心筋梗塞後もしくは心筋症や重症弁膜症などによる低左心機能症例に合併するが,可動性血栓の場合や塞栓症の既往がある場合,さらに心機能が改善傾向にある場合などは積極的外科手術の適応となる.今回,急性心筋梗塞後に左室心尖部血栓を生じた1例と,うっ血性心不全治療中に左室心尖部血栓を生じた1例に対して,右第4肋間から,完全内視鏡下に経左房・経僧帽弁アプローチで外科的血栓除去術を行い,良好な結果を得たので報告する.
We describe a 54-year-old man with neurofibromatosis type 1 who presented left-sided neck mass. Computed tomography demonstrated left common carotid artery aneurysm (51 × 33 mm). Surgery was performed because of the risk rupture. The found to be huge aneurysmal dilatation, and arterial wall partially collapsed extensively adherent surrounding tissues. 症例は66歳,女性.幼少期より左下肢の過成長傾向を認め,20歳台から浮腫や血栓性静脈炎などの静脈うっ滞症候群を繰り返していた.56歳時のCT検査で左外腸骨動脈瘤とリンパ管および静脈奇形を指摘され,今回66歳で左鼠径部の皮下腫瘤増大と同側股関節可動制限を自覚したため,精査加療目的で当科紹介となった.術前精査では,リンパ管および静脈奇形を主とする混合型血管奇形を合併した左外腸骨動脈瘤を認めた.左総腸骨動脈から下肢末梢動脈の拡張蛇行を認め,今回は破裂の危険性が高い直径43mmの外腸骨動脈瘤を治療対象とした.リンパ管および静脈奇形が左下腹部体壁から後腹膜にかけてびまん性に存在するため開腹瘤切除術は困難と判断し,浅大腿動脈アプローチのステントグラフト内挿術を施行した.術後は問題なく経過し,第7病日に退院となり,初回外来CT検査でもendoleak無く経過した.
症例1,67歳女性.劇症型心筋炎に対してbridge to recovery目的に両心補助人工心臓装着術を施行した.術前に中等度大動脈弁逆流を認め,生体弁(CEP Magna Ease 21 mm)による大動脈弁置換術を併施した.心機能の回復が得られたため,術後180日目に左心補助人工心臓離脱術を行った.補助人工心臓抜去後の体外循環離脱に際し,高度の左心機能低下を認め体外循環離脱困難に陥った.経食道心臓超音波で生体弁の高度狭窄が判明した.再度心停止.大動脈弁位の生体弁は,左室側から増生したパンヌスにより弁尖が癒合し高度狭窄の所見を呈した.パンヌスの剥離・除去により生体弁機能は回復し,体外循環から容易に離脱可能となった.症例2,68歳女性.劇症型心筋炎に対して両心補助人工心臓装着術および大動脈弁置換術(CEP 19...

症例は70歳の女性.脳出血による右片麻痺のため長期臥床状態,日常生活動作は全介助の状態であった.4年前に肺塞栓症で外科的血栓除去術を施行された.1年前に心不全症状があり,前医にて収縮性心膜炎と診断され,薬物治療をされていた.今回,発熱を主訴に前医を受診し,心エコーで心嚢液貯留を認め,心タンポナーデを疑われ当科紹介入院となった.心嚢穿刺ドレナージで膿性の排液があり,化膿性心外膜炎の診断となった.膿瘍の培養からはBacteroides fragilisが検出された.抗菌薬治療による保存的加療では軽快せず,左肋間小開胸による心膜開窓術を施行した.術後経過は良好で胸水培養も陰性であった.化膿性心外膜炎は難治性で予後も不良な疾患である.今回,Bacteroides fragilisによる化膿性心外膜炎という稀な症例に対して左肋間小開胸によるアプローチを選択し,良好な成績を得られたので報告する.
