症例は 16 歳女性.難治性吃逆と嘔吐が 4 週間持続した後,嗄声,四肢の異常感覚,複視,回転性めまいが出現した.MRI では延髄背側に異常信号を認め,血清中抗アクアポリン 抗体陽性が判明し,視神経脊髄炎関連疾患 (neuromyelitis optica spectrum disorder: NMOSD) の最後野症候群 (area postrema syndrome: APS) と診断.ステロイドパルス療法により症状は改善した.NMOSD は小児では比較的稀であるが,小児の難治性の吃逆や嘔吐症例においても NMOSD の APS は重要な鑑別疾患の 1 つと考えられた.
