Subpontic osseous hyperplasia (SOH) is a growth of bone occurring on the edentulous ridge beneath pontics fixed partial dentures (FPDs). 術前明らかな心疾患がなく,術前検査も異常のない72歳の男性患者が頸椎椎弓形成術を受け,術中に突然心電図上R on Tが出現し,多形性心室性頻拍へ移行した.リドカインにより洞調律に戻ったが手術を中止した.血液ガス,電解質,心エコーは正常,術後の安静時及び負荷心電図も異常を認めず安定していたが,術後10日目の早朝に心臓突然死した.剖検では有意の冠動脈閉塞がないにもかかわらず新鮮な心筋梗塞を認め,冠動脈スパスムの関与が強く疑われた.術中のエピソードに関しては,冠灌流圧の低下,エフェドリンの使用,浅麻酔等が冠動脈スパスムを誘発する要因になったと推測された.
