Ehlers-Danlos症候群(EDS)はコラーゲンやその修飾酵素の遺伝子異常により結合組織脆弱性を来す遺伝性疾患群である.血管型EDSはIII型コラーゲン遺伝子変異により動脈や管腔臓器,肺が脆弱化し突然の破裂を来す常染色体優性疾患であり,EDSのうちで最重症とされる.今回我々は,死後に血管型EDSの確定診断がついた1例を報告する.症例は60代男性で,左気胸,腸管穿孔と3回の腹部動脈瘤破裂歴があった.胸痛を主訴に救急外来を受診し,右多発肋間動脈瘤破裂に伴う血胸と大量喀血を認めた.動脈塞栓術により活動性出血を停止させ,開胸血腫除去術により気道出血を停止させた.しかし,右心不全増悪により29病日に死亡した.死亡直前に皮膚生検と採血を行い,専門機関にて血管型EDSの確定診断を得た.動脈瘤破裂,消化管穿孔,血気胸の既往があれば,血管型EDSの可能性を考慮し,その特徴を踏まえた迅速な対応が必要になる.
