Denosumab has been shown to reduce new vertebral, nonvertebral, and hip fractures in postmenopausal women with osteoporosis. In subjects who were treatment-naïve or previously treated alendronate, denosumab was associated greater gains bone mineral density (BMD) decreases turnover markers when compared alendronate-treated subjects. This trial designed compare the efficacy safety of risedronate over 12 months transitioned from daily weekly alendronate treatment considered be suboptimally...
We administered risedronate, a potent oral bisphosphonate, to patients with mild primary hyperparathyroidism in order 1) determine if we could normalize the serum calcium concentration short term, and 2) analyze changes homeostatic mechanisms responsible for maintaining hypercalcemia this patient population. When 7 days, risedronate reduced fasting concentrations without significant toxicity hyperparathyroidism. The decrease was accompanied by evidence of inhibition bone resorption, as...
High bone mineral density on routine dual energy X-ray absorptiometry (DXA) may indicate an underlying skeletal dysplasia. Two hundred fifty-eight individuals with unexplained high mass (HBM), 236 relatives (41% HBM) and 58 spouses were studied. Cases could not float, had mandible enlargement, extra bone, broad frames, larger shoe sizes increased body index (BMI). HBM cases harbour genetic disorder.High is a sporadic incidental finding DXA scanning of apparently asymptomatic individuals....
ABSTRACT High bone mass (HBM) can be an incidental clinical finding; however, monogenic HBM disorders (eg, LRP5 or SOST mutations) are rare. We aimed to determine what extent is explained by mutations in known genes. A total of 258 unrelated cases were identified from a review 335,115 DXA scans 13 UK centers. Cases assessed clinically and underwent sequencing anabolic loci: (exons 2, 3, 4), LRP4 25, 26), 1, the van Buchem's disease [VBD] 52-kb intronic deletion 3′). Family members for...
Objective: to assess whether specialist osteoporosis nurses delivering training care home staff can reduce fractures and improve the prescription of treatments versus usual care. Design: pragmatic cluster randomised controlled trial (RCT) with randomisation at Primary Care Organisation (PCO) level. Setting: homes (residential, nursing EMI) across England Wales within PCOs. Participants: all 300 PCOs in were invited take part. Of these, 58 agreed participate gained ethical approval time start...
Bisphosphonates are a safe and effective treatment for Paget's disease of bone, but little information is available about the factors influencing duration remission so obtained. We assessed 60 patients with treated disodium pamidronate (APD). The mean was 9.5 months (range 3-25). major influences were initial pretreatment alkaline phosphatase (ALP; r = -4.6, p < 0.0001), minimum posttreatment ALP (r -0.51, rate response bone turnover to first dose APD 0.61, 0.0001). Multiple linear...
Post hoc analysis of FRAME and ARCH revealed that on-study nonvertebral vertebral fractures by Month 12 were less common in women initially treated with romosozumab versus placebo or alendronate. Recurrent fracture risk was also lower romosozumab‑treated patients, there no fracture‑related complications. Results support continuing treatment post‑fracture.
Measurements of bone density (BMD) are central to the World Health Organisation (WHO) approach definition osteoporosis. Dual energy X- ray absorptiometry (DXA) remains gold standard technique for measuring mineral
We describe a case where extensive investigations were utilized to identify the aetiology of hypophosphataemia in patient presenting with non-specific symptoms. The diagnosis hyperventilation-induced was eventually established. Hyperventilation is relatively common cause isolated hypophosphataemia, but easily overlooked as cause.
Objective. Vertebral fractures (VFs) are frequently under-recognized, reflecting their lack of diagnostic clinical features. For example, although VFs associated with back pain, this is also common in the general population. To establish whether pain can be used to recognize patients VF, we investigated site people and without using a simple tool.
