症例は68歳,男性.胸腔鏡補助下にて右下葉切除とリンパ節郭清を施行した.術後2日目にドレーンを抜去,3日目より右前胸部に狭窄音が出現した.CT検査では右上葉,中葉気管支の狭窄及び右上葉に浸潤影,すりガラス影を認め,上葉が背側,中葉が腹側に位置していた.また,中下葉間を切離した際の中葉のステープルラインを上中葉間に認めた.気管支鏡検査では右上葉及び中葉気管支入口部に軽度の狭窄を認め,中葉捻転を疑い再手術を施行した.中葉は気管支を軸に90度時計方向に捻転,上葉が背側,中葉が腹側に位置しており中葉捻転と診断,捻転解除を施行した.下葉切除後にも中葉捻転が起こりうること,中葉捻転では中葉以外の部位に異常を認める場合があることを認識しておくことが重要であると考えられた.また,診断にはステープルラインの位置に着目すること,治療方針決定には造影CT検査が有用であると考えられた.
