症例は29歳の男性, 2日前に腹痛を自覚し他院受診. 急性膵炎の診断にて, 入院加療となる. 入院2日後に重症急性膵炎の悪化と診断され, 当センターへ緊急搬送となった. 来院時, 重症急性膵炎 (CT grade 2点, 予後因子4点) に増悪. 血清トリグリセリド (TG) 1,300mg/dLと異常高値, 尿量減少, 酸素化の悪化を認めた. 当院来院時は発症3日目で高TG血症が持続していたため, 急性腎障害, 呼吸不全を伴う高TG血症による膵炎の重症化と診断し, 一般的な急性膵炎治療・インスリン投与・高脂血症薬の投与に加えて, 血漿交換ならびに血液濾過透析を3日間, その後血液濾過透析単独療法を5日間実施し, 重症急性膵炎の良好な改善を得た. 高TG血症に起因する膵炎では, 重症化時に血漿交換療法が有効とされ, 本症例でもその早期実施が膵炎治療に奏効したと考えられた.
