【目的】両側上小脳動脈硬膜枝を流入動脈とした小脳テント硬膜動静脈瘻の1 例を報告する.【症例呈示】74 歳男性,呂律困難,嘔吐で発症.小脳出血,くも膜下出血を認め,両側上小脳動脈硬膜枝を流入動脈とした小脳テント硬膜動静脈瘻からの出血と診断した.シャントポイントが正中側に存在したため,まず経動脈的塞栓術を施行し,シャントの残存に対して,外科的手術を追加し,根治を得た.【結論】本症例は両側上小脳動脈硬膜枝のみを流入血管とする極めて稀な硬膜動静脈瘻であることが手術的にも確認された.上小脳動脈硬膜枝の認知の重要性と血管内治療の可能性について考察する.

我々が行っている急性期脳梗塞の予後改善を目指した基礎・臨床研究とその展望を紹介する.1)血栓回収療法普及プロジェクト(RESCUE-Japan Project):本治療の有効性は確立したが,未だ十分に普及していない.このため,全国調査の結果をもとに,治療の推進に取り組んでいる.2)救急隊用脳卒中病型予測スコア開発:脳梗塞患者の予後改善のためには迅速かつ適切な搬送が必須である.我々が開発したこのアプリケーションは診断率が高く,救急搬送に応用していく.3)急性期脳梗塞および頸部・頭蓋内動脈狭窄に対する脂質低下療法に関する臨床試験:スタチンを用いた研究をベースにPCSK-9 阻害薬を用いた研究を開始する予定である.4)急性期脳梗塞に対する細胞療法の基礎・臨床研究:梗塞後の機能回復を目指した再生医療研究として,骨髄単核球,傷害誘導性多能性幹細胞,羊膜由来間葉系幹細胞を用いた臨床研究を行う予定である.
