腸間膜膿瘍は憩室穿孔や炎症性腸疾患,癌などにより二次性に生じることが多く,特発性と診断される症例は比較的少ない.今回われわれは,特発性腸間膜膿瘍の症例を経験したため報告する.症例は77歳の男性で既往に糖尿病と肺動脈血栓症があり,3年前に当院で食道癌の切除歴がある.来院8時間前からの心窩部痛を主訴に,プレショック状態で救急搬送された.CTで小腸間膜内の大量のairを認め,穿孔性腹膜炎を疑い緊急手術を施行したところ,淡血性の大量腹水と小腸間膜内の膿瘍と気腫を認め,腸間膜膿瘍と診断し洗浄ドレナージを行った.生検した腸間膜の病理は炎症所見で,腸間膜生検部と血液培養で大腸菌が検出された.周術期には敗血症・脳梗塞・腸管皮膚瘻などをきたしたが,術後181日目に退院した.本症例では憩室や癌,炎症性腸疾患の併発なく特発性腸間膜膿瘍と診断した.原因は糖尿病による免疫力低下や食道切除後の低栄養によるbacterial translocationや腸間膜の血流障害が考えられた.
