Pneumothorax associated with thoracic endometriosis (TE) generally occurs in women around 30 years old and it usually affects the right pleural cavity. We herein report two cases of TE left-sided pneumothorax young women. In the present study, ClC-3, one of voltage-dependent chloride channels, was identified in human nasal tissue. situ hybridization and immunohistochemical investigations demonstrated localization ClC-3 serous acini ductal portions submucosal glands, which are primary source secretion. Our data suggest that this channel contributes to secretion via transport. Its dysfunction might lead abnormal such pathological states as sinusitis.

症例は50歳,女性.2013年5月に右下腹部痛を自覚.近医にて大腸内視鏡検査を施行したところ上行結腸に輪状潰瘍を認め,腸結核が強く疑われた.6月下旬,急激な腹痛を主訴に当院に来院し,イレウスと診断され緊急入院・緊急手術となった.腸結核が疑われた上行結腸の狭窄部を含めた腸管切除術を施行した.切除標本で腸結核による狭窄部に椎茸がはまり込みイレウスを呈しているのが確認された. 腸結核の患者を診察する際は,常にイレウスの危険性を念頭に置いた早期治療が重要であり,さらには狭窄の強い症例では食事指導も必要と考えられた.

患者は59歳女性で,34歳時,左B領域の乳癌で乳房切除・腋窩リンパ節郭清を受けた.T1(1.0×0.8cm),N0,M0,病期I,充実腺管癌であった.乳癌術後13年,血中CEAが14ng/mlと上昇,CTで肝S4に大きさ3.5cmの多血性の腫瘤がみられた.乳癌肝転移の診断で肝左葉切除を施行した.摘出標本では5個の腫瘍がみられ,いずれも肝鎌状間膜の近傍に存在した(S4;2個,大きさ3.5cm,1.2cm,外側区域:3個,大きさ0.5-0.7cm).病理検索で肝腫瘍は乳癌の肝転移と診断され,ER(+),PgR(+)であった.術後CEF療法,エキセメスタン内服を行い,肝切除後12年再発なく健在である.原発乳癌の部位がB領域で,すべての肝転移巣が肝鎌状間膜近くに存在しており,転移経路として乳房から腹直筋を通って肝鎌状間膜近傍の肝臓へ至るリンパ路を介したリンパ行性肝転移の可能性が考えられた.
