Hematoma arising within an intrapancreatic accessory spleen (IPAS) is extremely rare pathological entity.We present the case of a 39-year-old man with acute abdominal pain.The patient was initially diagnosed as pancreatic cystic neoplasm according to CT and MRI imaging.Distal pancreatectomy conducted because possibility malignancy.Surgical resection showed that lesion hematoma in IPAS.Our indicated differential diagnosis IPAS should be born mind for cases tail pancreas epidermoid cyst (ECIAS).
