Emerging evidence suggests that repetitive transcranial magnetic stimulation (rTMS) enhances cognition in mild cognitive impairment (MCI). Accelerated intermittent theta burst (iTBS) rTMS protocols are promising as they substantially reduce burden by shortening the treatment course, but safety, feasibility, and acceptability of iTBS have not been established MCI.
Background: Published results of our Phase I safety and feasibility trial accelerated intermittent theta burst stimulation (a-iTBS) in mild cognitive impairment (MCI) due to Alzheimer's disease showed a large effect-size improvement cognition. Objective: Further demonstrate target engagement by identifying whether changes local network-level functional connectivity relate the observed improvement. Methods: Eighteen patients with MCI received 3-day a-iTBS (8 sessions/day) left dorsolateral...
Abstract Freezing of gait (FOG) in individuals with Parkinson’s Disease is associated a loss automaticity. This automaticity demonstrated by worsening performance while dual tasking. Functional connectivity between the cerebellar vermis and cortex have previously been spatiotemporal measures PD. The objective this study was to determine whether corticocerebellar as measured task interference PD FOG. 55 participants were recruited (38 FOG, 17 non-FOG controls) undergo resting-state functional...

Synaptic loss is present in Alzheimer's disease and correlates with the severity of dementia. Loss synapses dementia Lewy bodies (DLB) does not correlate as clearly cognitive status its cause unclear. To begin to understand relationship between cognition synaptic DLB, we assessed immunoreactivity for synaptic-terminal specific protein, synaptophysin, hippocampus 14 DLB cases. Quantitative data were obtained using an Image-Pro semiautomated analysis system. We determined Braak stage,...
Objectives Electroconvulsive therapy (ECT) has demonstrated efficacy in treating core symptoms of Parkinson's disease (PD); however, widespread use ECT PD been limited due to concern over cognitive burden. We investigated the a newer technology known have fewer side effects (right unilateral [RUL] ultra-brief pulse [UBP]) for treatment medically refractory psychiatric dysfunction PD. Materials and methods This open-label pilot study included 6 patients who were assessed motoric, cognitive,...
A 59-year-old Caucasian man with a past history of Parkinson's disease (PD) status post-bilateral subthalamic nucleus (STN) deep brain stimulation (DBS), who also had treatment-resistant (TR) obsessive–compulsive disorder (OCD), and depression (TRD), presented for further evaluation management his TR OCD. After an unsuccessful attempt to treat OCD by reprogramming existing STN DBS, he was offered bilateral ventral capsule/ventral striatum (VC/VS) DBS surgery. In addition the expected...
Freezing of gait (FoG) is a common and debilitating condition in Parkinson's disease (PD) associated with executive dysfunction. A subtype FoG does not respond to dopaminergic therapy may be related noradrenergic deficiency. This pilot study explores the effects atomoxetine on PD patients dopa-unresponsive using novel paradigm for objective assessment. Ten were enrolled this eight-week open label study. Assessments included an exploratory analysis protocol that quantified spatiotemporal...
Recent studies demonstrated that progranulin plays an integral role in the pathogenesis of frontotemporal dementia. To begin to explore dementia with Lewy bodies, we investigate its association pathologic proteins characterize this disease. We assessed immunoreactivity for medial temporal lobe structures 12 cases bodies. Similar data were collected β-amyloid burden, and α-synuclein pathology. Blinded investigators used a 0-3—point scale quantify burden. Double labeling was also performed....
