Re-operative mitral valve surgery in patients with poor ventricular function can be challenging especially the presence of patent bypass grafts. We report case 11 severe ischemic cardiomyopathy who underwent reoperative repair through a limited right thoracotomy approach, on non-fibrillating beating heart.
