<h3>Objective</h3> To compare the efficacy of tacrolimus (TAC) and mycophenolate mofetil (MMF) for initial therapy lupus nephritis (LN). <h3>Study design</h3> This is an open randomised controlled parallel group study. <h3>Methods</h3> Adult patients with biopsy-confirmed active LN (class III/IV/V) were to receive prednisolone (0.6 mg/kg/day 6 weeks tapered) in combination either TAC (0.06–0.1 mg/kg/day) or MMF (2–3 g/day) months. Good responders shifted azathioprine maintenance. The primary...

The absence of accepted patient-centered outcomes in research can limit shared decision-making peritoneal dialysis (PD), particularly because PD-related treatments be associated with mortality, technique failure, and complications that impair quality life. We aimed to identify patient caregiver priorities for PD, describe the reasons their choices.

Shared decision making in patients with glomerular disease remains challenging because outcomes important to remain largely unknown. We aimed identify and prioritize caregivers describe reasons for their choices., setting, participants, & measurementsWe purposively sampled adult from Australia, Hong Kong, the United Kingdom, States. Participants identified, discussed, ranked focus groups using nominal group technique; a relative importance score (between zero one) was calculated. Qualitative...

Abstract Background While peritoneal dialysis (PD) can offer patients more independence and flexibility compared with in-center hemodialysis, managing the ongoing technically demanding regimen impose a burden on caregivers. Patient empowerment strengthen capacity for self-management improve treatment outcomes. We aimed to describe patients’ caregivers’ perspectives meaning role of patient in PD. Methods Adult receiving PD (n = 81) their caregivers 45), purposively sampled from nine units...

We have shown recently that spinal cord injury (SCI) decreases basal gastric contractions 3 days after injury. In the present study we used [(13)C]-octanoic acid breath test and strain gauges with aim to investigate time-course of recovery from postinjury stasis in rats underwent experimental SCI at level third thoracic (T3) vertebra. Following verification [(13)C]-breath sensitivity uninjured rats, conducted our experiments T3-spinal contusion (T3-CI), transection (T3-TX) or laminectomy...

Background and objective Residual renal function (RRF) correlates with mortality morbidity rates in patients receiving peritoneal dialysis (PD). We examined the effect of a biocompatible PD solution (Gambrosol Trio; Gambro Lundia AB, Lund, Sweden) lower concentrations glucose degradation products on decline RRF. Design, setting, participants, measurements Incident at 2 centers Canada 1 Hong Kong were randomized (by minimization) an open-label parallel group trial to receive Gambrosol Trio or...

To assess the efficacy of using ultrasonography (USG) in monitoring progress exit site infection (ESI) patients undergoing continuous ambulatory peritoneal dialysis (CAPD).Twenty-two cases newly diagnosed ESI and 20 with normal sites as controls were assessed by USG. The reassessed USG after finishing a course antibiotic therapy, sonographic findings correlated clinical outcome.Out 22 ESI, 21 had definite sonolucent zones around external cuffs, while one case findings. Of control sites, 16...

Peritoneal dialysis (PD) can offer patients more autonomy and flexibility compared with in-center hemodialysis (HD). However, burnout - defined as mental, emotional, or physical exhaustion that leads to thoughts of discontinuing PD is associated an increased risk transfer HD. We conducted the current study to compare incidence and risk factors of arterial thrombosis in lupus non-lupus primary glomerulonephritis. identified patients whom nephritis glomerulonephritis were diagnosed between 1993 2003 using our cohort database pathology registry. analyzed cumulative new thromboembolic events since diagnosis by Kaplan-Meier plot, studied multivariate analysis. 162 with 181 After a mean observation 8.1 years, 47 (14%) died, 23 (7%) lost follow-up, 38 (11%) developed 42...

Gastric reflexes are mediated mainly by vago-vagal reflex circuits in the caudal medulla. Despite fact that brainstem circuitry remains intact after spinal cord injury (SCI), patients with SCI at cervical level most often present gastric stasis an increased risk of reflux and aspiration contents. Background Gastric motility studies are frequently conducted with anaesthetized animal models. Some on the same species have reported differences in vagal control of stomach that could not be explained solely by slightly different experimental conditions. A possible limitation comparison between similar relates to use anaesthetic agents. Furthermore, effects may also limit generalizations mechanistic gastric function and conscious animals. We have shown recently that our model of experimental high-thoracic spinal cord injury (T3-SCI) mirrors the gastrointestinal clinical presentation neurotrauma patients, whereby T3-SCI animals show diminished gastric emptying and dysmotility. In this study we used cholecystokinin as a peptide to test hypothesis induced gastroparesis is due, in part, an impaired vagally-mediated response peptides. We measured responses sulfated (CCK-8s) control (3 or 21 days after injury) rats utilizing: (i)...
