Sudden death is one of the more frequent causes for hemodialysis patients, but underlying mechanisms, contribution arrhythmia, and associations with serum chemistries or dialysis procedure are incompletely understood. To study this, implantable loop recorders were utilized continuous cardiac rhythm monitoring to detect clinically significant arrhythmias including sustained ventricular tachycardia, bradycardia, asystole, symptomatic in patients over six months. Serum tested pre- post-dialysis...
Point of care ultrasound (POCUS) is being explored for dynamic measurements like inferior vena cava collapsibility index (IVC-CI) and left ventricular outflow tract velocity time integral (LVOT-VTI) to guide anesthesiologists in predicting fluid responsiveness the preoperative period treating post-induction hypotension (PIH) with varying accuracy.
Rationale & ObjectiveThe incidence of arrhythmia varies by time day. How this impacts individuals on maintenance dialysis is uncertain. Our objective was to quantify the relationship with day and timing dialysis.Study DesignSecondary analysis Monitoring in Dialysis study- a multicenter prospective cohort study.Settings ParticipantsLoop recorders were implanted for continuous cardiac monitoring 66 participants follow up 6 months.ExposureTime based 6-hour intervalsOutcomesEvent rates...
Parachute mitral valve and Pacman heart (incomplete muscular ventricular septal defect) are rare congenital deformities usually reported in infants children. Very few adult patients with these anomalies but the association of two has not been described. This report describes a 56-year-old male exertional dyspnea who was detected to have moderately severe regurgitation stenosis. Typical parachute deformity reduced opening common attachment all chordae single posteromedial papillary muscle...
Atrial fibrillation (AF) is common in patients with kidney failure on hemodialysis (KF-HD). We determined both AF incidence and burden KF-HD using implantable loop recorder (ILR) monitoring
Hypoplastic right heart syndrome is a rare cyanotic congenital disease with under-development of the ventricle, tricuspid, and pulmonary valves leading to right-to-left shunting blood through inter-atrial septal defect. Perinatal mortality high very few patients surviving adulthood without corrective surgery. This report describes 26-year-old young woman, who had recurrent abortions stillbirths detected have marked cyanosis hypoplastic heart, sub-arterial ventricular defect, absent valve,...
A 23-year-young female presented with mild exertional dyspnoea and palpitation since early childhood. By deploying 2D- 3D echocardiography, she was detected to have situs solitus, atrioventricular ventriculoarterial discordance L-malposition of great vessels, valvular pulmonary stenosis, large secundum atrial septal defect, bicuspid aortic valve, right-sided arch, moderately severe mitral tricuspid valve regurgitation. Typical parachute deformities the morphologically valves were observed....
Clopidogrel has been the only available antiplatelet drug used along with aspirin in patients of ACS. In recent years 2 new drugs (Prasugrel and Ticagrelor) have become available. Prasugrel dose 10 mg OD found to be more efficacious but increased risk major bleeding. For this reason it not gained widespread usage ACS undergoing PCI. There are no systematic data on use Indian population. This is a prospective, multicentric, hospital registry 1000 PCI who were administered Prasugrel. The...
Congenitally unguarded tricuspid valve (TV) orifice, a variant of TV dysplasia, is rare malformation with protean manifestations. This report describes symptomatic adult male gross right heart failure and atrial fibrillation, who was found to have an orifice isolation the trabecular apical cavity ventricle (RV) muscular ridges separating outflow tract (forme-fruste double-chambered RV). The ventricular remained patent.
Abstract Background/Methods No studies have examined safety of a no fasting (NF) strategy as compared to current nil per os (NPO) practice in patients undergoing non emergent cardiac catheterization (CC). We these two strategies this single center, prospective, randomized study. Fasting group was NPO for solids after midnight but could clear liquids up 2 hours prior procedure. NF had restriction or oral intake. Results A total 520 enrolled till December 2018 were included interim analysis....
Congenital anomaly wherein the mitral valve leaflets are directly attached to papillary muscle(s) (PM) with or without short under-developed chords is rarely reported in adults. Patients two PMs an intervening fibrous bridge have also been included under this head previous studies. Echocardiography enables accurate evaluation of morphology and function leaflets, chordae tendineae, PM. This report describes a series six patients aged 56–84 years who had abnormal large solitary anomalously...
We report a case of very late stent thrombosis 7 years post sirolimus eluting implantation presenting as ST elevation MI while on dual antiplatelet therapy. Angiography revealed an aneurysm at the proximal end stent. The patient was managed successfully by primary percutaneous coronary intervention (PCI) with adjunct thrombus aspiration and intracoronary abciximab administration followed deploying mesh-covered MGuard. This complication is rare presentation after drug illuting (DES).
Although acquired left ventricular outflow obstruction has been reported in a variety of conditions, there are scant reports its occurrence following mitral valve replacement (MVR). This study describes two female patients, who developed severe discrete subaortic stenosis, five years MVR. In both cases, the was replaced by porcine Carpentier-Edwards 27-mm bioprosthesis with preservation leaflets. The risk very late tract after bio-prosthetic MVR subvalvular apparatus needs to be kept mind...
Mitral annular calcification (MAC) has been considered a predisposition and an association of thrombo-embolic disease. Superimposed thrombus on MAC is under-appreciated as potential cause systemic thrombo-embolism. This report describes elderly gentleman, who had recurrent cerebral embolism in one the episodes, large mobile was detected ventricular surface calcified mitral annulus. The disappeared after initiation anti-coagulation.
The study was planned to compare Anti-thrombotic strategies for patients undergoing PCI in a real world population with an emphasis on occurrence of major bleeding, composite ischemic end points and economic outcomes.
Post induction hypotension (PIH) is a largely debated topic in anaesthesia , without any definitive consensus on ideal method for its accurate identification. We hypothesized that inferior venae cava collapsibility index (IVCCI)measured pre-operatively before general could predict this .The patients were categorized the basis of their American Society Anaesthesiologists physical status classification(ASA) to find optimal cutoff value and sensitivity predicting PIH.In prospective...
Aortic regurgitation (AR) usually occurs in diastole presence of an incompetent aortic valve. Systolic AR is a rare phenomenon occurring patients with reduced left ventricular systolic pressure and atrial fibrillation or premature contractions. Its occurrence Doppler peculiarity adds to the hemodynamic burden.Rheumatic carditis often characterised by acute subacute severe mitral (MR) due flail anterior leaflet elongated chords. In MR, developed may fall mid late systole afterload...
Doppler-derived trans-prosthetic gradients are higher and the estimated effective valve area is smaller than catheter-derived directly measured hemodynamic values, mostly due to pressure recovery phenomenon. Pressure a varying extent common all prosthetic heart valves including bioprostheses. recovery-related differences usually small except in patients with bileaflet metallic prosthesis, wherein high-pressure local jets across central orifice have been documented since long back also narrow...
Development of left ventricular pseudo-aneurysm following mitral valve replacement is rare. Subannular pseudoaneurysm due to separation the annulus from free wall excessive traction or debridement has been reported since long. Aneurysm formation excision papillary muscles and leaflets spontaneous mid-ventricular rupture thin ventricle rarely reported. This report describes a middle-aged woman who developed heart failure three months after replacement. She was detected have pseudo-aneurysms...
