患者女,57岁。因发现左乳肿块1周余于2018年7月入院,行左乳单纯切除术及前哨淋巴结活检术。乳腺肿块大小2.5 cm×1.5 cm,与周围组织界限不清,切面囊实性,含棕褐色胶冻样物质。镜下观察见多个导管囊性扩张,囊腔内充满黏液。囊腔内衬上皮呈单层、复层及复杂分支乳头状结构。瘤细胞高柱状,胞质内富含黏液,细胞核位于基底,核异型性轻到重度。在肿瘤主体附近可见普通型浸润性导管癌和高级别导管原位癌成分。免疫组织化学黏液性肿瘤细胞CK7弥漫强阳性,雌激素受体、孕激素受体、HER2阴性。未检测到BRAF V600E及KRAS第12、13和61位密码子突变。病理诊断为乳腺黏液性囊腺癌。乳腺黏液性囊腺癌罕见,需排除卵巢和胰腺的黏液性囊腺癌转移。.

二代测序等分子生物学技术的应用使得人们对子宫间叶源性肿瘤有了新的认识。本文将介绍延胡索酸水合酶-缺陷型平滑肌瘤、PGR重排上皮样平滑肌肉瘤、PLAG1重排黏液样平滑肌肉瘤、SMARCA4缺失未分化子宫肉瘤、血管周上皮样细胞肿瘤、类似卵巢性索间质肿瘤的子宫肿瘤、炎性肌纤维母细胞肿瘤、伴NTRK融合或COL1A1-PDGFB融合的子宫肉瘤等新近认识的子宫间叶源性肿瘤,重点讨论其临床病理学及分子遗传学特征。相关分子遗传学改变对肿瘤的精准诊断、预后评估和靶向治疗有重要应用价值。.
