症例は54歳,女性.主訴は腹痛.1994年に子宮体癌に対し子宮・付属器摘出術,傍大動脈リンパ節郭清,および術後補助化学療法を施行された.2007年9月に腹痛を認め近医で腹部CTを施行したところ,膵頭部前面に腫瘤性病変を認め,精査加療目的に当院を紹介された.画像上,径約3cmの辺縁平滑な楕円形の腫瘤が膵頭部と胃前庭部の間に存在し,内部は壊死性変化をきたしていた.膵との間には脂肪組織が介在するものの胃壁との境界が不明瞭な部位が存在した.FDG-PETでは同部位にのみ集積を認めた.胃gastrointestinal stromal tumorもしくは腸間膜腫瘍を疑い,腹腔鏡下腫瘍摘出術を施行した.術中所見で腫瘍は右胃大網動脈起始部に存在し,胃,膵との連続は認めなかった.病理組織診断では子宮体癌のリンパ節転移であった.今回われわれは子宮体癌術後13年目に右胃大網動脈領域リンパ節再発をきたした1例を経験したので,文献的考察を加えて報告する.

症例は49歳の男性で,胃検診で異常を認めたため精査を行ったところCTで胃大彎側のリンパ節腫大を伴う胃前庭部大彎と連続した12×8 cmの腫瘤を認めた.胃由来のgastrointestinal stromal cell tumor(以下,GISTと略記)の診断で手術を施行した.腫瘍は胃前庭部大彎より発生しており,右胃大網静脈領域(No6およびNo14v)のリンパ節腫大を認めた.リンパ節を術中迅速診断に提出したところCastleman病(Castleman’s disease;以下,CDと略記)の診断であったため胃局所切除術を行って手術を終了した.術後4か月後のCTでは再発所見を認めずリンパ節は消失していた.CDはリンパ濾胞の過形成に加え形質細胞が濾胞間に多数浸潤する組織所見を有する原因不明のリンパ増殖性疾患である.CDを合併したGISTの症例はこれまで報告がない.
