症例は81歳の女性で,上部消化管出血による出血性ショックから多臓器不全を発症し集中治療を受けた.全身状態が改善した後に施行された上部消化管内視鏡検査で幽門部に隆起性病変の存在が確認され,生検でgroupVが検出された.各種画像検査で胃の軸捻転と胸腔内への逸脱が確認されたためにupside down stomachに併発した早期胃癌と診断され,腹腔鏡下に幽門側胃切除術を行った.術後に逆流性食道炎の発症が懸念されたため再建はRoux-en-Y法が選択された.最終病理組織診断はtype0-IIc+IIa pT1apN0M0,ly0,v0,Stage IAだった.Upside stomachを伴う早期胃癌に対しても腹腔鏡下手術は有用な選択肢の一つであると考えられた.
