65歳女性,COVID-19肺炎に対し気管挿管と腹臥位療法を施行した.酸素化が改善し抜管を試みたが,吸気性喘鳴の聴取と痰の喀出が不十分なため再挿管となった.5日後に再度抜管を試みたが,発声困難と痰の喀出が不十分なため再び挿管となった.短期間での抜管は困難と判断し気管切開を施行した.気管切開後17日目に気管カニューレを抜去したが呼吸困難を生じカニューレを再挿入した.喉頭内視鏡検査,頚部CT検査から披裂軟骨脱臼が診断された.脱臼整復後も症状の改善が見られず,両側喉頭麻痺の合併が考えられた.長期挿管や腹臥位療法施行時には,披裂軟骨脱臼や喉頭麻痺にも注意するべきである.
