There is minimal information on coronavirus disease 2019 (COVID-19) in immunocompromised individuals. We have studied 10 patients treated at 12 adult care hospitals. Ten kidney transplant recipients tested positive for severe acute respiratory syndrome 2 (SARS-CoV-2) by polymerase chain reaction, and 9 were admitted. The median age was 57 (interquartile range [IQR] 47-67), 60% male, 40% Caucasian, 30% Black/African American. Median time from to COVID-19 testing 2822 days (IQR 1272-4592)....
We compared the outcome of COVID-19 in immunosuppressed solid organ transplant (SOT) patients to a naïve population. In total, 10 356 adult hospital admissions for from March 1, 2020 April 27, were analyzed. Data collected on demographics, baseline clinical conditions, medications, immunosuppression, and course. Primary was combined death or mechanical ventilation. assessed association between primary prognostic variables using bivariate multivariate regression models. also endpoint SOT an...
Abstract Background Several studies have shown that transplanting a hepatitis C virus (HCV)‐negative recipients with HCV‐positive donor is feasible in research setting. In February 2018, we began HCV‐negative donors as standard of care. Methods All patients, except those previously cured HCV and cirrhosis, were consented for NAT‐positive kidneys. After transplantation, patients tested RNA until viremic. A direct‐acting antiviral (DAA) agent was prescribed based on genotype insurance...
Abstract Introduction This study examines whether the use of inpatient Continuous Glucose Monitors provides improved glycemic control over finger‐stick glucose monitoring post‐transplant. Methods is a single‐site, prospective randomized controlled trial 40 patients receiving conventional or continuous using Medtronic Guardian Sensor 3 during first 5 days Included were adult renal transplant recipients with diagnosis diabetes. Assessed endpoints included post‐transplant daily median level,...
Abstract The beta‐amyloid peptide (Aβ) is considered responsible for the pathogenesis of Alzheimer’s disease. Despite magnitude reports describing a neurotoxic role extracellular Aβ, intracellular Aβ (iAβ) has not been elucidated. We previously demonstrated that in rat pheochromocytoma cells expression moderate levels results up‐regulation phospho‐extracellular signal‐regulated kinases (ERK1)/2 along with an elevation cyclic AMP‐response element (CRE)‐regulated gene expression; however,...
Deceased donor kidneys with acute kidney injury (AKI) and donation after circulatory death (DCD) are viable sources of organs. The outcomes renal transplantation from DCD donors AKI not known.A retrospective review deceased transplants performed 2006 to 2016 was conducted using the United Network for Organ Sharing dataset. Donors were stratified by or brain dead status stage. Recipients followed until graft failure end study. Cox regression used adjust donor, recipient, transplant covariates...
Many strategies regarding timing of native nephrectomies exist for patients with symptomatic polycystic kidney disease (PCKD).This is a single-center, retrospective study 594 adults PCKD who had renal transplants from 1994 to 2014. Three groups were analyzed: transplant-only recipients (tx alone), simultaneous bilateral and transplant (simultaneous), pretransplant (pre). The primary outcome was graft survival. Secondary outcomes included postoperative complications.Five hundred sixty-five...
Human Herpes Virus-8 (HHV-8) may reactivate in immunocompromised patients including recipients of solid organ transplants. Reactivation HHV-8 result Kaposi sarcoma (KS). KS typically occurs with dermatologic involvement but can affect virtually any other organ; most commonly the gastrointestinal tract. We present a diagnostically challenging case South American woman 7 months after kidney transplant. She presented recurrent urinary tract infection manifested by pelvic pain and dysuria....
Abstract Background The clinical course of SARS‐CoV‐2 in the pediatric kidney transplant population is not well described. Methods We performed a retrospective cohort study at New York center. Baseline characteristics and patients with positivity (Ab or PCR) were described, comparison between COVID‐positive COVID‐negative was performed. Results Twenty‐two had COVID‐19 IgG testing performed, eight whom also PCR testing. 23% our evidence infection. Four positive only, one patient PCR. All five...
This case series reviews four critically ill patients infected with severe acute respiratory syndrome coronavirus 2 (SARS - CoV 2) [coronavirus disease 2019 (COVID-19)] suffering from pneumatosis intestinalis (PI) during their hospital admission. All received the biological agent tocilizumab (TCZ), an interleukin (IL)-6 antagonist, as experimental treatment for COVID-19 before developing PI. and TCZ have been independently linked to PI risk, yet cause of this relationship is unknown under...
Kaposi sarcoma (KS) following kidney transplantation can result from recipient reactivation of latent human herpesvirus 8 (HHV-8) infection or activation donor-acquired HHV-8 infection. Post-transplant KS typically manifests with cutaneous pathology, but rare cases renal allograft involvement have been reported. We describe two donor-derived in hepatitis C (HCV) viremia-negative transplant recipients who each received a donor HCV viremia. One did not develop while the other presented acute...
Literature supports equivalent kidney transplant outcomes in adults with systemic lupus erythematosus (SLE) compared those without SLE. However, there are conflicting and scant data on outcomes, as well controversy over optimal timing of transplantation, children adolescents SLE.Analysis included kidney-only recipients aged 2-21 years from 2000 to 2017 enrolled the Organ Procurement Transplant Network (OPTN). The relationship between diagnosis (SLE n = 457, non-SLE glomerular disease 4492,...
Collapsing glomerulopathy (CG) is a severe form of which results in nephrotic syndrome and often ensues rapid progression to end-stage kidney disease (ESKD).Although most commonly result HIV infection, other conditions such as parvovirus B19 (PB19) infection have been associated with CG.We present case an 18-year-old male CG PB19 who was heterozygous for APOL1 G1 G2 genetic variants. In attempt treat, he started on intravenous immunoglobulin (IVIg), however rapidly progressed ESKD.During...
Race coefficients of estimated glomerular filtration rate (eGFR) formulas may be partially responsible for racial inequality in preemptive listing kidney transplantation. We used the Scientific Registry Transplant Recipients database to evaluate differences distribution before and after application Modification Diet Renal Disease (MDRD) Chronic Kidney Epidemiology Collaboration (CKD-EPI) race all preemptively listed non-Black transplant candidates (eGFR modulation). Odds were calculated by...
Graft macrosteatosis can predispose to a higher risk of graft loss so we sought redefine acceptable cutoffs for steatosis.Data 26,103 donors who underwent liver transplantation (LT) between January 2004 and December 2018 from the UNOS-STAR database were utilized. A high-risk steatotic (HRS) low-risk (LRS) defined as ≥20% <20% macrosteatosis, respectively. High-risk grafts further classified with 20-29% (G1S grafts), 30-39% (G2S ≥40% steatosis (G3S grafts). Outcomes groups compared.LRS had...
Liver transplantation necessitates life-long immunosuppression, increasing the risk of infection. Compared with viral and bacterial infections, post-transplant fungal infections are rare invasive aspergillosis (IA) constitutes a fraction all infections. IA is typically result
Lymphatic leakage is a common and well-described complication after kidney transplantation, occurring in up to 25% of patients. Accumulation lymph due the surgical disruption recipient lymphatic channels accompanying external iliac vessels, complicated by lower extremity edema, wound breakdown, infection, and, if unresolved, graft loss extrinsic compression. In this report, we describe novel use diagnostic therapeutic lymphangiography successfully treat leak renal transplant that was...
In many instances, kidney transplants cannot be performed because of incompatibility (such as blood type or cross-match) among donors and recipients. One established way to address this finding is by means donor exchanges, where incompatible pairs are enrolled in a registry. Registries can lead paired exchanges chains. chains, non-directed (altruistic) initiates sequence that culminates with so-called bridge-donor. Studies have estimated triggers on average 4.8 8 (if O) (1). Although usually...
Abstract The kidney allocation system (KAS) is based on quality-based “longevity matching” strategies that provide only a momentary snapshot of expected outcomes at the time transplantation. purpose our study was to define continuous timeline relative and mutual interactions donor recipient characteristics graft survival. Total 39,108 subjects who underwent transplant between October 25, 1999 January 1, 2007 were identified in United Network for Organ Sharing dataset. Our primary outcome...
Background. Existing literature has demonstrated the significant relationship between race and kidney transplant outcomes; however, there are conflicting limited data on influence of donor or donor-recipient race-matching pediatric outcomes. Methods. Analysis included kidney-only recipients ages 2 17 from 2000 to 2017 enrolled in Organ Procurement Transplantation Network their associated donors. Multivariable regression models were used compare outcomes by race-matched status. Results. Of...
In this case report, we describe the clinical course of a complicated transplant renal artery (TRA) pseudoaneurysm, clinically featured by gross and massive hematuria one month after kidney was performed on 50 year-old male patient. TRA pseudoaneurysm is rare but potentially life-threatening complication that may result in bleeding, infection, graft dysfunction/loss, lower limb ischemia/loss, hemorrhagic shock, death. treatment remains challenging as it needs to be tailored patient...
