To examine the safety and pharmacokinetics of clinical response to leflunomide, a de novo pyrimidine synthesis inhibitor, when administered patients with active rheumatoid arthritis (RA) who have been receiving long-term methotrexate therapy. This was an open-label, 52-week study in which 30 RA that remained despite therapy at 17+/-4 mg/week (mean +/- SD) for > or =6 months were given 10-20 mg/day. Patients assessed adverse effects, pharmacokinetic measurements leflunomide methotrexate, by...
High-dimensional single-cell analyses have improved the ability to resolve complex mixtures of cells from human disease samples; however, identifying disease-associated cell types or states in patient samples remains challenging because technical and interindividual variation. Here, we present mixed-effects modeling associations single (MASC), a reverse association strategy for testing whether case-control status influences membership any multiple cellular subsets while accounting...
Abstract Introduction Despite recent advances in anti-inflammatory therapy, rheumatoid arthritis (RA) patients continue to rate pain as a priority. The etiology of RA is likely multifactorial, including both inflammatory and non-inflammatory components. In this study, we examine the association between disease activity, sleep, psychiatric distress sensitivity RA. Methods Fifty-nine female completed questionnaires underwent pressure threshold testing assess hyperalgesia/allodynia at joint...
The COPA syndrome is a monogenic, autoimmune lung and joint disorder first identified in 2015. This study sought to define the main pulmonary features of an international cohort patients, analyse patient responses treatment highlight when genetic testing should be considered. We established subjects (N=14) with seen at multiple centres including University California, San Francisco, CA, USA. All had one previously mutations gene, clinically apparent disease arthritis. analysed...
Diclofenac sodium, a phenylacetic acid—derived nonsteroidal anti-inflammatory drug (NSAID) recently released in the United States, was associated with development of significant hepatitis seven patients, one death. Signs and symptoms developed within several weeks initiation use generally resolved 4 to 6 following discontinuation drug. The only patient rechallenged recurrence her hepatic abnormalities. In patient, fatal, fulminant despite early withdrawal Review European literature disclosed...
To determine the effect of disclosure rheumatoid arthritis (RA) risk personalized with genetics, biomarkers, and lifestyle factors on health behavior intentions. We performed a randomized controlled trial among first-degree relatives without RA. Subjects assigned to Personalized Risk Estimator for Rheumatoid Arthritis (PRE-RA) group received web-based PRE-RA tool RA factor education estimates, including genotype/autoantibody results behaviors (n = 158). comparison arm standard 80). The...
To ascertain the frequency of polymyalgia rheumatica (PMR) with a normal erythrocyte sedimentation rate (ESR; < or = 30 mm/hour) and to determine any defining clinical characteristics.A retrospective chart review study all patients meeting criteria for PMR seen over 5-year period in hospital an office-based rheumatology practice. We evaluated 117 patients; 26 (22.2%) had pretreatment ESR mm/hour (mean +/- SD 19.8 7.5 versus 74.4 30.3 elevated group; P<0.0001). Of patients, 15 (58%) were...
In a matter of weeks, the COVID-19 crisis has totally upended clinical medicine and practice rheumatology. Our old ways seeing patients in person, precepting fellows clinic or on consult service, leading in-person interactive teaching rounds are gone for now possibly good. But how can we replace these critical training elements? Despite mounting challenges managing growing population COVID-19–infected caregivers, must find innovative to foster our trainees' professional development. The...
Abstract Twelve patients with refractory rheumatoid arthritis were treated total lymphoid irradiation (TLI) to a cumulative dose of 3,000 rads. Post‐TLI morbidity/mortality included 8 xerostomia, 4 weight loss >10 kg, 3 or more teeth, herpes zoster, bacterial infection that was fatal in 2, hypothyroidism, 1 cutaneous vasculitis, and death from myocardial infarction patient cardiorespiratory arrest another. Ten the reevaluated 15–40 months (mean ± SE, 30 2) after completion TLI,...
A type II collagen-specific arthritogenic lymphokine has been identified in the rat. Arthritogenic factor (AF) is a 65 kD protein generated vitro by T cells from rats with collagen arthritis, and it induces an erosive, proliferative synovitis when injected into knee joint of syngeneic naive recipients. Complement does not appear to be required. These data identify potential cell-mediated effector mechanism this model, suggest that AF may function other inflammatory synovial diseases.
Abstract Objective. To evaluate the efficacy of intravenous immune globulin (IVIG) in treatment stiff‐man syndrome (SMS). Methods. An open, unblinded study 3 patients with active disease and/or refractory to diazepam corticosteroids. Results. All bedridden improved substantially shortly after infusion IVIG and regained function. Conclusion. may be useful for SMS.
Objective. We evaluated the T-SPOT. TB assay to identify latent tuberculosis infection (LTBI) in patients with rheumatic disease receiving immunosuppressive medication including tumor necrosis factor (TNF) antagonists. Methods. A total of 200 seen Arthritis Center at Brigham and Women’s Hospital were enrolled for study. Most US-born women rheumatoid arthritis. medical history was obtained using a questionnaire, whole blood drawn assay, tuberculin skin testing (TST) performed. Results. Both...
To determine the frequency of weight loss in patients treated with leflunomide for rheumatoid arthritis at an referral center. We queried 35 rheumatologists Robert Breck Brigham Arthritis Center to if had occurred as adverse event between November 1998 and January 2000. Five such were identified their clinical course was reviewed.Five 70 who begun therapy significant that could not be linked other identifiable etiologies. The amount substantial this group patients, ranging from 19 pounds 53...

Before the group meeting, sent health care collect perceptions about barriers adoption gather input on best potential strategies addressing these barriers. The panel consisted of 5 managed pharmacists 3 physician experts rheumatology, dermatology,...
Objective Remission is a key goal in managing rheumatoid arthritis (RA), with sustained remission as the preferred sequelae of short-term remission. However little known about predictors for patients reaching Using two independent cohorts, we aimed to evaluate prevalence and Methods The study cohort consisted subjects RA from Brigham Women's Hospital Rheumatoid Arthritis Sequential Study (BRASS) Korean Observational Network (KORONA). We analyzed who reached 2009 follow up data consecutive...
