透析関節症の疼痛を, 長期にわたりコントロールする目的で, ステロイドと柴苓湯の併用を試みた. 対象は透析歴8年以上で3箇所以上の慢性の関節痛をきたし, 非ステロイド系消炎鎮痛剤が無効な症例7人とした. ただし二次性上皮小体機能充進症の症例, 慢性関節リウマチなどの多発性関節痛を誘発する疾患を持つ症例は除外した. まずプレドニン®2.5-10mgとツムラ柴苓湯®6gの連日投与で開始し, 以後プレドニン®を1-2週間毎に漸減した. 5週以降は関節痛の抑制が可能であればステロイドは中止し, 柴苓湯単独での治療を目指した. 疼痛の程度は, 患者自身の自己評価で無痛を1とし, 最大の痛みを10として, 整数で10段階で評価する疼痛スコアで表現した. ステロイド・柴苓湯併用療法を施行する前は疼痛スコア平均8.2±1.6であったが, 併用後では疼痛スコア2.3±1.7となり全例で十分な除痛効果がみられた. しかも, 多くは併用開始後1週間以内に鎮痛効果を認めた. ステロイド離脱1か月後の柴苓湯単独投与の際にも, 疼痛スコア3.6±2.2とほぼ満足できる鎮痛効果が得られた....
