目的 探讨微型钛板螺钉联合克氏针内固定治疗近、中节指骨粉碎性骨折的临床疗效。 方法 回顾性分析2012年2月—2015年10月上海交通大学附属第一人民医院整形外科手术治疗的近、中节指骨粉碎性骨折55例(58指)患者的临床资料,其中近节34例36指、中节21例22指;拇指近节6指,余四指近节30指、中节22指。51指采用微型钛板螺钉内固定治疗,7指应用微型钛板螺钉联合克氏针内固定治疗;指骨背侧固定42例,侧方固定16例。 结果 术后随访6~15个月,平均随访10个月。所有患者伤口Ⅰ期愈合,术后未发现钛板螺钉松动、变形或断裂者,无一例发生骨折不愈合和关节僵硬。骨折均达到解剖复位;骨折愈合时间10~24周,平均17周。按照TAM标准评价术后手指功能:优37指、良18指、差3指,优良率为94.8%。 结论 采用微型钛板螺钉联合克氏针治疗近、中节指骨粉碎性骨折,操作简便、创伤小、固定可靠,术后可以早期功能锻炼,利于手指功能的康复。
