Mechanistic insights from 3D echocardiography (echo) can guide therapy. In particular, ischemic mitral regurgitation (MR) is difficult to repair, often persisting despite annular reduction. We hypothesized that (1) in a chronic infarct model of progressive MR, parallels changes the geometry leaflet attachments, causing increased tethering and restricting closure; therefore, (2) MR be reduced by restoring toward normal, using new ventricular remodeling approach based on echo findings.We...

Background: Although clinical studies have suggested a link between inflammation markers and atrial fibrillation (AF), it is still unclear whether local immunologic responses actually exist in human atria during AF. Methods Results: To address this point, left appendages were obtained from 16 patients who underwent cardiac surgery (5 with sinus rhythm (SR) 11 AF) subjected to immunohistochemical analysis. In all the AF specimens, adhesion migration of CD45-reactive cells consistently...

Background— To improve the longterm outcome after CABG, several strategies have been used using arterial conduits. Our 20 years experience with right gastroepiploic artery (GEA) graft was evaluated. Methods and Results— In 1352 patients having CABG GEA graft, (1092 men, mean 63 years, 99% multivessel disease, EF 0.51), internal thoracic artery, saphenous vein, radial grafts were concomitantly in 1312 (97%), 783 (58%), 128 (8%) patients, respectively. The number of distal anastomoses 3.1, 2.4...

Purpose To predict radiation pneumonitis (RP) grade 2 or worse after lung stereotactic body therapy (SBRT) using dose‐based radiomic (dosiomic) features. Methods This multi‐institutional study included 247 early‐stage nonsmall cell cancer patients who underwent SBRT with a prescribed dose of 48–70 Gy at an isocenter between June 2009 and March 2016. Ten dose–volume indices (DVIs) were used, including the mean dose, internal target volume size, percentage entire excluding receiving greater...

Purpose: To quantify the predictive uncertainty in infrared (IR)‐marker‐based dynamic tumor tracking irradiation (IR Tracking) with Vero4DRT (MHI‐TM2000) for lung cancer using logfiles. Methods: A total of 110 logfiles 10 patients who underwent IR Tracking were analyzed. Before beam delivery, external markers and implanted gold monitored 40 s camera every 16.7 ms an orthogonal kV x‐ray imaging subsystem 80 or 160 ms. model [four‐dimensional (4D) model] was then created to correlate positions...

Abstract Adiponectin receptors, AdipoR1 and AdipoR2 exert anti-diabetic effects. Although muscle-specific disruption of has been shown to result in decreased insulin sensitivity exercise endurance, it remains be determined whether upregulation could reverse them obese diabetic mice. Here, we show that expression human increased levels genes involved mitochondrial biogenesis oxidative stress-detoxification almost the same extents as treadmill exercise, concomitantly endurance Moreover,...

Purpose: To present the dosimetric characterization of a multileaf collimator (MLC) for new four‐dimensional image‐guided radiotherapy system with gimbaled x‐ray head, MHI‐TM2000. Methods: MHI‐TM2000 has an head composed ultrasmall linear accelerator guide and system‐specific MLC. The can rotate along two orthogonal gimbals (pan tilt rotations) up to ±2.5°, which swings beam ±41.9 mm in each direction from isocenter on plane perpendicular beam. MLC design is single‐focus type, 30 pairs 5...

Antigen-specific regulation of T cell response is crucial for limiting hyperimmune response. In this study, we discovered that antibodies specific to the antigen peptide-MHC class II complex are produced during helper responses various antigens. These specifically inhibited receptor (TCR) recognition MHC molecules presenting peptide. We have termed these ′immune- induced TCR-like antibodies′ (iTabs). Immunization with peptides containing flanking residues iTabs, whereas immunization lacking...

A poor functioning dilated left ventricle with mitral regurgitation is the worst condition in chronic ischemic heart failure. Our 7-year experience combined valve and ventricular reconstruction was evaluated.Among 246 patients having undergone a ventriculoplasty for postinfarction dysfunction our experience, there were 76 advanced failure due to cardiomyopathy (70 males 6 females mean age of 60 years). All had NYHA class III (n = 41) or IV 35) failure, including 26 (34%) inotropic support...

Abstract The human adiponectin receptors, AdipoR1 and AdipoR2, are key anti-diabetic molecules. We previously reported the crystal structures of revealing that their seven transmembrane helices form an internal closed cavity (the form). In this study, we determined structure D208A variant AdipoR1, which is fully active with respect to major downstream signaling. Among three molecules in asymmetric unit, two assume form, other adopts open large openings cavity. Between closed- open-form...
