症例は67歳,女性.黄疸を主訴に近医受診,磁気共鳴胆道膵管造影にて下部胆管閉塞を認め当院消化器内科に紹介.CTで下部胆管に20mm大の腫瘍を認め,胆管は閉塞していた.膵頭部背側に腫大したリンパ節を認めた.内視鏡的逆行性胆道膵管造影では胆管内に突出する内部不均一,境界明瞭な腫瘤を認めた.ポジトロンCTでは下部胆管の腫瘤にSUVmax=5.6の集積を認め,CTで指摘されたリンパ節も軽度集積を認めた.下部胆管癌の診断で亜全胃温存膵頭十二指腸切除術を施行したところ,膵頭部に白色腫瘤を認め,その上流の胆管は著明に拡張していた.またリンパ節#13に転移を認めた.組織像では腫瘍細胞は胞巣状~シート状に配列し,周囲組織に浸潤していた.免疫染色に加え,MIB-1 indexおよび核分裂像から,2010年WHO分類においてNET G2に相当する胆管原発の神経内分泌腫瘍と診断した.
