Among 43 monozygotic (MZ) and 19 dizygotic (DZ) pairs in which an index case had definite Parkinson9s disease (PD), only one MZ pair was definitely concordant for PD. When with questionable clinical features were included, 4 of 48 1 DZ concordant. The frequency PD cotwins cases similar to that expected unrelated control group matched age sex. Although we unable identify a single environmental agent, conclude the major factors etiology are nongenetic.
Background and Purpose —We sought to define an effective safe dose of botulinum toxin type A (Dysport) for the treatment upper limb muscle spasticity due stroke. Methods —This was a prospective, randomized, double-blind, placebo-controlled, dose-ranging study. Patients received either placebo or 1 3 doses Dysport (500, 1000, 1500 U) into 5 muscles affected arm. Efficacy assessed periodically by Modified Ashworth Scale battery functional outcome measures. Results —Eighty-three patients were...
Task scheduling is essential for the proper functioning of parallel processor systems. Scheduling tasks onto networks processors an interesting problem that well-defined and documented in literature. However, most available techniques are based on heuristics solve certain instances very efficiently reasonable amounts time. This paper investigates alternative paradigm, genetic algorithms, to without need apply any restricted assumptions problem-specific, such case when using heuristics....
In comparison with closely matched controls, patients Parkinson's disease had reduced scores in tests of odor detection and qualitative discrimination. Olfactory impairment was not related to age, duration symptoms, treatment, intellectual function, or genetic factors seemed be a nonmotor manifestation the disease.
Many value-added and content delivery services are being offered via service level agreements (SLAs). These can be interconnected to form a overlay network (SON) over the Internet. Service composition in SON has emerged as cost-effective approach quickly creating new services. Previous research addressed reliability, adaptability, compatibility issues for composed However little been done manage generic quality-of-service (QoS) provisioning services, based on SLA contracts of individual In...
The clinical and behavioural features of 5 patients examined during transient global amnesia (TGA) are described. All underwent extensive neuropsychological testing sequentially after the attack. Our results show that in TGA there is a characteristic deficit which parallels permanent amnesic syndrome. Personality, complex cognition problem solving, semantic knowledge, language visuospatial function remained intact. Immediate memory was preserved. Longer-term verbal nonverbal severely...
The risk of people with Parkinson's disease (PD) falling is greater than that the general population but to date, disease-specific predictors have not been identified. To identify one or more features, which would predict individuals at during a 3-month prospective follow-up study.A battery standardised tests administered in home and laboratory telephone interview.Sixty-three PD were recruited from GP practices. Eleven interview variables six gait used subsamples (55 44 subjects,...
To explore the perceived life and identity changes described by individuals following a single stroke using narrative approach.Individuals admitted to hospital with stroke, no previous disability, returning home; took part in interviews hospital, six months one year post-discharge. The Gross Motor subscale of Rivermead Assessment Nottingham 10 point Activities Daily Living Scale were completed.Eight respondents (five male, three female; mean age 67 years (range 56-82). motor score was 9...
It has become increasingly desirable for companies worldwide to outsource their complex e-business infrastructure under the utility computing paradigm by means of service level agreements (SLAs). A successful provider must be able not only satisfy its customers' demand high service-quality standards, but also fulfill commitments based upon business objectives (e.g., cost-effectively minimizing exposed impact violations). This paper presents design rationale a business-objectives-based SLA...
The purpose of this paper is to introduce the Life Thread Model, which incorporates established psychological and social theory related identity change following an acquired disability. It supported by a growing body empirical evidence can be used broaden our understanding service provision in rehabilitation. We suggest that limited appreciation processes underpinning rehabilitation has led different agendas for patients professionals, lack recognition power relationships, negative views...
Twenty-four parkinsonian patients compared pergolide and bromocriptine therapy in a randomized double-blind, two-period crossover study. Both drugs were adjusted to an optimal balance between benefits side effects. The mean daily dose range for 3.3 mg (0.7 7.2) 42.7 (5.8 87.5), respectively. Adjunctive medications, which most included levodopa (plus carbidopa), not altered during the A similar spectrum of clinical effects was found with both lisuride, used treat 13 previous Despite...
Machine vision for plant phenotyping is an emerging research area producing high throughput in agriculture and crop science applications. Since 2D based approaches have their inherent limitations, 3D analysis becoming state of the art current technologies. We present automated system analyzing growth indoor conditions. Objective: To evaluate the validity and reliability of an English version Impact on Participation Autonomy Questionnaire (IPA). The original Dutch IPA has been shown to load onto five factors. Design: A validation study. Setting: Outpatients clinics people’s homes. Subjects: Two hundred thirteen people with multiple sclerosis, rheumatoid arthritis, spinal cord injury, general practice attendees, stratified by level disability (median age 54, 42% male, 58% female). Inclusion criteria: as...
To explore the views of therapists, nurses and doctors working in a variety rehabilitation settings on goal-setting process.Sixteen staff from three different attended workshop which difficulties associated with were described their potential solutions.Five identified. (1) Formal was felt to be an activity unusual for many patients although intrinsic professional groups. (2) Goal-setting is often insensitive people's roles community. (3) hospital environment does not transfer easily (4)...
Twenty-eight parkinsonian patients were studied in a double-blind, crossover comparison of lisuride and bromocriptine. All but two completed the study, with each drug adjusted to an optimal dose (mean daily intake 4.5 mg for 56.5 bromocriptine). Treatment was given 7 10 weeks; three assessments made at biweekly intervals levels. Conventional antiparkinsonian medications, including lev- odopa, not changed. Efficacy adverse effects assessed by objective subjective techniques. The only...
Abstract Background Liver disease is within the top five causes of premature death in adults. Deaths caused by complications cirrhosis continue to rise, whilst deaths related other non-liver areas are declining. Portal hypertension primary sequelae and associated with development variceal haemorrhage, ascites, hepatic encephalopathy infection, collectively termed decompensation, which leads hospitalisation mortality. It remains uncertain whether administering a non-selective beta-blocker...
The deprenyl and tocopherol antioxidative treatment (DATATOP) study has shown that selegiline (deprenyl), with or without tocopherol, reduces physical psychological deficits in patients Parkinson9s disease within one month of the probability reaching a primary endpoint, decision to treat levodopa. This paper critically re-evaluates inference neuroprotective effect thus delays progression disease. Analysis is based on simple model which assumes clinically measured impairment proportional...
There was strong evidence of a neurological explanation for transient feelings compulsion reported by three patients. In at least two cases the mechanism epileptic. The frontal lobe implicated in all three. A feeling compulsion, divorced from action, has rarely if ever been epilepsy or other disorders.
We study dependable group communication for large-scale and delay-sensitive mission critical applications. The goal is to design a protocol that imposes low loads on bottleneck network links provides both stable throughput fast delivery of multicast messages even in the presence frequent node link failures. To this end, we propose our GoCast protocol. builds resilient overlay proximity aware has balanced degrees. Multicast propagate rapidly through an efficient tree embedded overlay. In...
We studied the actions of lisuride, a dopaminergic ergot derivative, in 20 parkinsonian patients. When dose was increased gradually, most patients tolerated up to 5 mg daily. Clinical assessment and objective, computer-assisted evaluation revealed improvement akinesia, rigidity tremor. Adverse reactions were similar those seen with levodopa bromocriptine, but somnolence tended occur more often lisuride.
