慢性活動性Epstein-Barr (以下EB ) ウイルス感染症 (chronic active Epstein-Barr virus infection; 以下CAEBV) は, EBウイルスの再活性化によるさまざまな病態の総称であり, 単なる感染症ではなくEBウイルス関連リンパ増殖症 (EBV-LPD) である. 症例は44歳の女性. 難治性の咽頭痛を主訴に複数の医療機関を受診し, 診断が確定しないため当科紹介となる. 右の口蓋扁桃と梨状陥凹に白苔を伴う潰瘍性病変と肝機能異常を認めた. EBウイルスの抗体価の上昇, EBウイルスDNAの上昇, 病理組織学的検査より口腔咽頭病変の原因はCAEBVによるものと診断した. 点滴および経口ステロイドの使用により潰瘍性病変の進行は停止し, 肝機能異常の改善, EBウイルスDNAの減少を認めた. CAEBVは, EBウイルスによるリンパ増殖性疾患としてとらえられており予後は不良であるため, 今後は血液内科での化学療法を予定した. 本症例を経験し難治性の口腔咽頭病変ではCAEBVも鑑別する必要があると考えた.
