50歳,女性。3年前より右後頭部に自発痛・脱毛斑を伴う小結節が出現し,増大した。病理組織学的にアポクリンへの分化を有する腺癌を認め,免疫染色ではCK7陽性かつCK20陰性,ER,PR,ARはすべて陽性,GCDFP-15陽性,p63陰性,D2-40陰性を示した。PETを含む全身検索では乳癌を含む他臓器癌を認めず,皮膚に原発したアポクリン腺癌と診断した。後頭部病変を拡大切除し1年経過したが,再発や転移は認めていない。皮膚原発アポクリン腺癌は乳癌の皮膚転移と病理組織学上鑑別困難であるため,最終的に臨床経過で診断するに至った。
