日本小児アレルギー学会・喘息死委員会宛てに1989~2017年10月までに気管支喘息に罹患していて死亡し登録された症例は新規登録6名を加えて252例で, 対象外を除いた219例について解析した. 男女比は136/83 (1.6/1) であった. 死亡年齢は0~3歳, 12~16歳の思春期にピークがみられた. 都道府県別では, 東京24例, 愛知16例, 福岡15例などの順となっていた. 喘息の型はアトピー型が47%で約半数を占め, 季節性では通年性が38%であった. 死亡前1年間の重症度は, 重症26%, 中等症18%, 軽症18%, 不明・未記入38%であった. 不明・未記入の割合が1999年までは34%に対し, 2000年以降は53%と増加していた. 入院回数は0回37%, 1回15%, 2回5%, 3回以上14%, 不明・未記入29%であった. 受診時の状況は, 受診時死亡例 (dead on arrival : DOA) が58.2%, 死因は窒息が59.6%でいずれも最も多かった.

乳幼児期は,気管支喘息の児に対し早期介入を行う上で重要な時期である.反復性喘鳴を生じる児では,この時期に気道のリモデリングや呼吸機能低下を生じ得るため,早期に治療を開始する必要がある.しかし,喘息の症状である喘鳴はこの年齢層ではありふれた症状であるため,急性喘鳴であれば喘鳴が生じた後の経過観察を続け,反復性喘鳴であれば他疾患の鑑別を行ったうえで,治療を開始する.診断にあたっては乳幼児喘息の診断フローに従い診断を進めるが,この時期の喘息の診断は困難であることも多く,重症度に応じた長期管理薬を使用することで診断的治療を行い,治療への反応性を評価し乳幼児喘息と診断することもできる.診断後は,IgE関連もしくは非IgE関連喘息を鑑別する.また,喘鳴を呈する他疾患との鑑別が重要であり,特に治療への反応が乏しい場合は喘息以外の疾患も考慮し診療する.
