症例は65歳の男性である.全身倦怠感が持続し,初発2カ月後より意識障害が緩徐に進行した.頭部MRIでは脳幹,前頭葉,側頭葉,基底核にFLAIR画像で高信号域をみとめ,Gdで増強されなかった.髄液中のEpstein-Barr virus(EBV)-DNA量の増加をみとめ,EBV感染がうたがわれた.アシクロビル投与に加えステロイドパルスを施行したが,効果は乏しく意識レベルはしだいに低下し,第146病日に死亡した.剖検所見は脳組織に大型異型細胞の増殖をみとめ,免疫染色ではEBER(EBV encoded small RNAs)陽性であり,加齢性EBV関連B細胞リンパ増殖性異常症による中枢神経浸潤と診断した.
