目的 了解广州市地区住院儿童呼吸道腺病毒感染的临床特点和流行病特征。 方法 收集了2011年1月至2014年12月间小儿呼吸道腺病毒感染的临床资料共544例,并进行了回顾性分析。 结果 临床诊断上呼吸道感染63例(11.6%),下呼吸道感染481例(88.4%)。在下呼吸道感染里,支气管炎87例(16%),非重症肺炎336例(61.8%),重症肺炎58例(10.7%)。混合感染241例,最常见混合病原是肺炎支原体134例(24.6%)。儿童呼吸道腺病毒感染最常见的临床症状是发热94.9%和咳嗽98%。男女感染比是2.7∶1,平均年龄是29月。0~6岁儿童是呼吸道腺病毒感染的好发年龄段(92.1%)。重症肺炎有7例(1.3%)死亡,其中对存活的51例重症腺病毒感染肺炎患儿随访发现17例(27.4%)出现闭塞性细支气管炎,2例(4%)出现支气管扩张,1例(2%)出现肺气肿。 结论 腺病毒感染是引起儿童呼吸道感染的重要病原学,重症腺病毒肺炎导致严重的肺部后遗症。
