ALK gene rearrangements are important oncogenic driver alterations in non-small-cell lung cancer and a predictive factor. When present, it enables the use of tyrosine kinase inhibitors, such as Alectinib. This drug can lead to anemia erythrocyte morphological changes. We present case 72-year-old patient diagnosed with stage IV non-small cell treated Alectinib first line. The was very effective reducing tumor size but G2 acanthocytosis side effects. Anemia improved once replaced by Brigatinib.
