会陰ヘルニアは,ほとんどが腹会陰式直腸切断術あるいは骨盤内臓全摘術後に発生する続発性会陰ヘルニアである.今回,われわれは腹会陰式直腸切断術後に発生した続発性会陰ヘルニアの1例を経験したので報告する.症例は67歳,男性.直腸癌(Rb,MPN1M0,cStage IIIa)に対し術前化学放射線療法を施行後,両側側方リンパ節郭清を伴う腹会陰式直腸切断術を施行した.術後経過は良好であったが,術後12カ月後頃より会陰部の膨隆が出現した.次第に膨隆は増大傾向し,術後18カ月目の腹部造影CT検査においても会陰部の小腸の脱出を認め,続発性会陰ヘルニアと診断した.術後21カ月目,経腹的にヘルニア修復術を施行した.術中所見として,骨盤底に約10cm大のヘルニア門を認めた.ヘルニア門を覆うようにComposix® mesh (15.2cm×20.3cm)を骨盤底に固定し,骨盤底再建術を施行した.
