症例は47歳の男性で,14歳時に十二指腸潰瘍に対して広汎胃切除,Billroth-II(B-II)再建を受けた.M蛋白血症で当院血液内科通院中,悪性リンパ腫除外目的に行われたFDG-PETにて胃に異常集積を指摘された.上部消化管内視鏡検査で残胃空腸吻合部口側にポリープ状隆起と不整な潰瘍性病変を認め,生検にて高分化型腺癌と診断された.CTではリンパ節転移や遠隔転移は認められず,残胃癌と診断し,開腹残胃全摘術を施行した.病理所見では,ポリープ状隆起は胃小窩の延長や幽門腺に類似した粘液腺の増生および囊胞化を認め,gastritis cystica polyposa(以下,GCPと略記)と見なされた.また,GCP表層部分において腫瘍細胞の核にEpstein-Barr virus(以下,EBVと略記)の感染を伴うリンパ球浸潤癌の像を認めた.今回,GCPを発生母地としたEBV関連リンパ球浸潤癌と考えられるまれな症例であったため報告する.
