In a prospective study involving 642 patients with febrile urinary tract infection (UTI), we found antimicrobial pretreatment (odds ratio [OR], 3.3), an indwelling catheter (OR, 2.8), and malignancy 2.7) to be independent risk factors for bacteremia uropathogen that was not cultured or recognized in the urine.
