食道胃接合部病変の代表的運動機能不全症である,食道裂孔ヘルニアとアカラシアの食道運動機能を,術前後の内圧測定により,比較検討した.また食道筋層の病理組織学的検討と,手術成績についても併せて評価した.その結果,以下のような結論を得た.食道裂孔ヘルニアによる逆流性食道炎の原因として,胃内圧の上昇と,第1次蠕動波の異常による下部食道の排出障害が大きく関与していると考えられた.手術によってlower esophageal sphincter(LES)の圧と長さは回復したが,蠕動運動障害は残存した.狭窄症状を有する例では,下部食道筋層のAuerbach神経叢に変性があり,Nissen法では有効な結果のえられないことがあった.食道アカラシアの術後LESの機能改善はえられたが,蠕動運動の障害は残った.アカラシアの病態病理として,myenteric plexusの変性の他に,神経線維や軸索の変性による神経伝達輸送の障害の関与も推察された.
