Abstract Purpose High‐quality CBCT and AI‐enhanced adaptive planning techniques allow CBCT‐guided stereotactic radiotherapy (CT‐STAR) to account for inter‐fractional anatomic changes. Studies of intra‐fractional respiratory motion management with a surface imaging solution CT‐STAR have not been fully conducted. We investigated in breath‐hold Ethos‐based CT‐SBRT (stereotactic body non‐adaptive radiotherapy) using optical combined onboard CBCTs. Methods Ten cancer patients mobile lower lung or...

PurposeHerein we report the clinical and dosimetric experience for patients with metastases treated palliative simulation-free radiation therapy (SFRT) at a single institution. Methods MaterialsSFRT was performed institution. Multiple fractionation regimens were used. Diagnostic imaging used treatment planning. Patient characteristics as well planning time points collected. A matched cohort of conventional computed tomography simulation (CTRT) acquired to evaluate differences in time. SFRT...

Abstract Background Online adaptive stereotactic radiotherapy allows for improved target and organ at risk (OAR) delineation inter-fraction motion management via daily planning. The use of SBRT the treatment pancreatic cancer (performed until now using only MRI or CT on rails-guided radiotherapy), has yielded promising outcomes. Herein we describe first reported case cone beam CT-guided (CT-STAR) cancer. Case presentation A 61-year-old female with metastatic presented durable palliation a...

A novel O-ring gantry can deliver stereotactic body radiation therapy (SBRT) with artificial intelligence-facilitated, CT-guided online plan adaptation. It gates mobile targets by optically monitoring skin surface motion. However, this gating solution has not been clinically validated. We conducted a trial to evaluate the feasibility of optical surface-guided for patients upper abdominal or lower thoracic malignancies treated SBRT on platform (NCT05030454).Ten who were prescribed target and...

Our institution was the first in world to clinically implement MR-guided adaptive radiotherapy (MRgART) 2014. In 2021, we installed a CT-guided (CTgART) unit, becoming one of clinics build dual-modality ART clinic. Herein review factors that lead development high-volume program and treatment census over an initial, one-year period. The clinical service at our is enabled with both MRgART (MRIdian, ViewRay, Inc, Mountain View, CA) CTgART (ETHOS, Varian Medical Systems, Palo Alto, platforms. We conducted a prospective pilot study evaluating the feasibility of same day MRI-only simulation and treatment with MRI-guided adaptive palliative radiotherapy (MAP-RT) for urgent indications (NCT#03824366). All (16/16) patients were able to complete 99% their first on-table attempted fractions, no grades 3-5 toxicities occurred.

Magnetic resonance (MR) image (MRI) guided radiation therapy (MRgRT) confers superior daily soft-tissue target visualization for many anatomic sites compared with traditional computed tomography (CT)–based and is increasingly used to manage inter- intrafraction motion in a variety of cancer diagnoses.1,2 There growing evidence that MRgRT may be improve the dosimetric therapeutic index treatment through improved setup accuracy, opportunity online plan adaptation, real-time cine MRI tumor...

Stereotactic body radiotherapy (SBRT) to the central and ultra-central thorax is associated with infrequent but potentially serious adverse events. Adaptive SBRT, which provides more precise treatment planning inter-fraction motion management, may allow delivery of ablative doses tumors effective local control improved toxicity profiles. Herein, we describe first reported case cone beam computed tomography (CBCT)-guided stereotactic adaptive (CT-STAR) in non-small cell lung cancer (NSCLC) a...

Spatially fractionated radiation therapy is hypothesized to allow for safe, hypofractionated, dose- escalated large tumors.1-3 can be delivered in a grid-like pattern (ie, GRID) using 3-dimensional conformal planning techniques. It also intensity modulated lattice therapy). Lattice delivers low dose gross tumor volume (GTV) with high spherical vertices spaced within the tumor, which has previously been described small case series of patients gynecologic and other malignancies.
