In previous studies, breast cancer patients with positive sentinel lymph node(s) (SLN) after neoadjuvant chemotherapy (NAC) frequently had additional nonSLN involvement. Per guidelines, residual SLN disease warrants completion axillary node dissection (cALND), which has increased morbidity. Given recent improvements in NAC, we hypothesized that positivity may be lower than previously reported for certain subgroups.
Abstract Introduction Since 2020, the Precision Medicine Initiative has provided no cost germline and testing to all patients with solid tumors. We sought measure impact of this initiative on mutation frequency in breast cancer (BC) across multiple social determinants health. Methods The electronic health record was queried for diagnosed BC between 2020 2024 at our Southern California clinical network sites who had through Initiative. Frequency pathogenic/likely pathogenic variants (P/LPV)...
Abstract Background: Trastuzumab deruxtecan (T-DXd) is an ADC consisting of anti-HER2 antibody linked to a novel topoisomerase I inhibitor payload approved for T-DM1-refractory, HER2+ breast cancer (BC) and metastatic, recurrent HER2-low BC. While this therapy effective in some patients, resistance often develops. To aid developing new therapies T-DXd-resistant BC better understand potential mechanisms, we established panel six XPDX models representing innate or acquired T-DXd. These models,...
Cost effective care requires comprehensive person-centred formulation of solutions. The East London NHS Foundation Trust Community Health Services in Newham have piloted models Integrated Care called 'Virtual Wards' which aim to keep people living with multiple long-term conditions, well at home by minimising system complexity. These Virtual Wards comprise Interdisciplinary Teams (IDTs) a General Practitioner (GP) seconded provide leadership. Historically assessments been dominated...
Abstract Purpose of Review Up to 10% all breast cancers are associated with inherited germline mutations. Genetic testing guidelines for hereditary cancer susceptibility have changed significantly since their inception in the late 1990s. The purpose this review is discuss changes technology that improved our ability efficiently detect mutations, introduce recent paradigm change towards population-based currently supported by American Society Breast Surgeons, and highlight challenges...
Abstract Purpose of Review This review summarizes the most recent data on management small, node-negative Her2+ and triple-negative breast cancer. Recent Findings Both cancers are characterized by high rates recurrence worse survival outcomes compared to hormone-positive cancers. De-escalation systemic therapy in early-stage cancer is a national trend clinical research. prospective trials support scaling back cytotoxic agents maximization targeted regimens. Similarly, large retrospective...
