Obstructive sleep apnea (OSA) is a risk factor for cardiovascular disease. We hypothesized that patients with OSA and no disease have oxidant-related microcirculatory endothelial dysfunction. To evaluate the microcirculation in OSA.This study included seven age- weight-matched control subjects (mean age, 38 yr; mean body mass index, 32.5 kg/m²). All participants were free of factors. Participants received measurement brachial artery flow-mediated dilation forearm subcutaneous biopsy. Patients...
BACKGROUND Lack of insurance after traumatic injury is associated with decreased use postacute care and poor outcomes. Insurance linkage programs enroll eligible patients in Medicaid at the time an unplanned admission. We hypothesized that enrollment would be increased care, but also prolonged hospital length stay (LOS) while awaiting authorization. METHODS linked trauma registry EMR data to identify ages 18 years 64 admitted from 2017 2021 a Level I center. Patients without retroactively...
Abstract Background Textbook outcomes are composite outcome measures that reflect the ideal overall experience for patients. There many of these in elective surgery literature but no textbook have been proposed patients following emergency laparotomy. The aim was to achieve international consensus amongst experts and best Outcomes non-trauma trauma Methods A modified Delphi exercise undertaken with three planned rounds regarding based on category, number importance (Likert scale 1–5)...
We aimed to identify socioeconomic and clinical risk factors for post-intensive care unit (ICU)-related long-term cognitive impairment (LTCI).After delirium during ICU stay, LTCI has been increasingly recognized, but without attention factors. We enrolled a prospective, multicenter cohort of survivors with shock or respiratory failure from surgical medical ICUs across 5 civilian Veteran Affairs (VA) hospitals 2010 2016. Our primary outcome was at 3- 12 months post-hospital discharge defined...
The novel coronavirus (COVID-19) pandemic has led surgical societies to recommend delaying diagnosis and treatment of suspected lung cancer for lesions less than 2 cm. Delaying can lead disease progression, but the impact this delay on mortality is unknown. COVID-19 infection rate at which immediate operative risk exceeds benefit We sought model versus delayed resection in a suspicious nodule cm.A decision analysis was developed, sensitivity analyses performed. base case 65-year-old male...
This study investigates the challenge posed by state borders identifying population, injury, and geographic scope of areas country where closest trauma center is out-of-state, collating emergency medical services (EMS) policies relevant to cross-border transport.
Importance: Socially Responsible Surgery (SRS) integrates surgery and public health, providing a framework for research, advocacy, education clinical practice to address the social barriers of health that decrease surgical access worsen outcomes in underserved patient populations. These patients face disparities both healthcare, which can be effectively addressed by surgeons collaboration with allied professionals. Objective: We reviewed current state populations American rural communities,...
Although timely access to trauma center (TC) care for injured patients is essential, the proliferation of new TCs does not always improve outcomes. Hospitals may seek TC accreditation financial reasons, rather than address community or geographic need. Introducing risks degrading case and payer mix at established TCs. We hypothesized that newly accredited would see a disproportionate share commercially insured patients.
Injured patients have high rates of uninsurance, which is associated with worse outcomes. Insurance linkage programs that connect to Medicaid coverage can prevent catastrophic costs for patients. Less known about the long-term impact insurance enrollment. We examined health care utilization previously uninsured patients, hypothesizing newly insured postinjury would use more than those remaining uninsured.
Objective Lung cancer is the leading killer in women, resulting more deaths than breast, cervical and ovarian combined. Screening for lung has been shown to significantly reduce mortality, with some evidence that women may have a greater benefit. This study demonstrates population of being screened breast greatly benefit from screening cancer. Methods Data 18,040 who were 2015 at two imaging facilities also performed reviewed. A natural language-processing algorithm followed by manual chart...
A key point that the UK General Medical Council addressed in its recommendations on undergraduate medical education was concept of 'core curriculum' (General 1993). Although enthusiastic for idea reducing factual overload, many teachers found themselves facing task how to define what a core curriculum is, should be included and why. Predictably, our initial response is include common important topics, but common, does one determine relative importance topics? We do not claim have unravelled...
This editorial is in response to the three latest clinical consensus guidelines authored by Critical Care Committee of American Association for Surgery Trauma. Herein, we discuss their main findings and recommendations impact on practice Surgical Care.
End-of-life (EOL) care is associated with high resource utilization. Recognizing and effectively communicating that EOL near promotes more patient-centered care, while decreasing futile interventions. We hypothesize provider assessment of futility during the surgical intensive unit (SICU) admission would result in higher rates Do Not Resuscitate (DNR).
