Percutaneous edge-to-edge devices for non-surgical repair of mitral valve regurgitation are under clinical evaluation in high-risk patients deemed not suitable conventional surgery. To address guidelines initial therapy decision, we here report on 13 cases surgery after failed percutaneous or attempted repair, and discuss methodology prognostic factors operative outcome this situation. Thirteen referred to our cardiothoracic unit using the technique, were treated surgically failure between...
Objectives To summarize our single Institution experience with staged total percutaneous management of aorto‐mitral pathology. Background Percutaneous treatment aortic valve stenosis (AVS) and mitral regurgitation (MVR) has been recently proposed for patients at high surgical risk. Methods Data concerning consecutive undergoing transcatheter AV implantation (TAVI) followed by MV repair MitraClip® were prospectively collected analyzed. Results From January 2010 to February 2012 a 254 referred...
The aim of this study was to compare treatment moderate severe symptomatic mitral regurgitation (MR) with either conventional surgery or the valve edge-to-edge device (MitraClip®) in very elderly patients. newly introduced MitraClip has demonstrated promising acute results treating patient cohort. Also noteworthy is fact that patients who otherwise would have been denied are increasingly referred for device. We sought review our institutional experience, comparing outcomes both surgical and...
Whipple’s disease is a rare, often multisystemic chronic infectious caused by the rod-shaped bacterium Tropheryma whipplei. Very rarely heart involved in process of disease, leading to culture-negative infective endocarditis. Up 20 % all endocarditis are blood and therefore diagnostic challenge. We present two unusual cases encountered different patients with prior history arthralgia. A rheumatic arthritis or even transient arthralgia should put whipplei on top differentials this age group...
To assess the prevalence of gastrointestinal neoplasia in patients with Streptococcus bovis infectious endocarditis we performed a retrospective cohort analysis all episodes S. treated at our institution between January 2000 through December 2014. Twenty-five were identified for this purpose. 12/25 received colonoscopy and 1/25 was assessed CT colonography. Of 13 who underwent colonic assessment, 11 diagnosed neoplasms different stages development. In absence any strong contraindication,...
Cardiac myxomas (CMs) are the second most common benign primary cardiac tumors, mainly originating within left atrium. Approximately 5% of CM cases associated with Carney Complex (CNC), an autosomal dominant multiple neoplasia syndrome often caused by germline mutations in protein kinase A regulatory subunit 1A (PRKAR1A). Data concerning PRKAR1A alterations sporadic variable and sparse, reported to range from 0% 87%. Therefore, we investigated frequency using next-generation sequencing...
A 48-year-old man presented to a neighbouring cardiology unit with epigastric pain sensation. Acute myocardial infarction (AMI) was suggested by laboratory parameters for and electrocardiogram showing ST-segment elevation in the inferior lateral leads ( Panel G ). Emergency coronary angiogram revealed subtotal occlusion of posterolateral branch …
The rapid-deployment valve system (RDVS) was introduced to facility minimally invasive aortic replacement. In this study we evaluate the potential benefits of RDVS in elderly high-risk patients with endocarditis valve.Since introduction our institution December 2017 through October 2021, EDWARDS INTUITY prosthesis (Model 8300A, Edwards Lifesciences, Irvine, CA, USA) has been implanted a total 115 for different indications by single surgeon. Out one-hundred and fifteen cases implantation,...
A 19-year-old athlete with sudden onset dyspnoea and progressive exercise intolerance presented to our Heart Centre for assessment of his cardiopulmonary function. grade 3/6 continuous murmur best heard along the lower left sternal boarder was only pathological finding upon physical examination. …
Background Patients scheduled for surgery after unsuccessful MitraClip® intervention present increasingly with multiple comorbidities, and they are often referred to the heart team suggest most appropriate intervention. The publication of successful results initial patient cohorts treated MitraClip device has resulted in recruitment more seriously ill patients, who otherwise would have been denied catheter-based/surgical treatment. There reports on conventional failed mitral valve repair...
A frequently underdiagnosed complication of pacemaker and implantable cardioverter defibrillator lead implantation is the unintentional advancement leads into systemic circulation. We report a case encountered in our clinic 70-year old man evaluated neighbouring with symptoms transient ischaemic attack initially unclear aetiology. Posterior–anterior chest X-rays suggested that was left heart. This finding confirmed by transthoracic transoesophageal echocardiography.
Bartonella quintana is a gram-negative microorganism that can lead to culture-negative infective endocarditis (IE) in immunocompromised patients. Here, we present an exceptionally rare case of 70-year-old male with quintana-associated IE primarily limited the tricuspid valve spread mitral after replacement. This was then complicated by spondylodiscitis thoracic vertebrae, ultimately resulting death due cardiac arrest.
Congenital factor VII (FVII) deficiency is a rare bleeding disorder (RBD) with phenotypes ranging from asymptomatic state to life threatening episodes. There no established recommendation for the perioperative management of patients scheduled cardiac surgery. We have described patient FVII treated aortic valve stenosis, coronary artery disease, and atrial fibrillation. Balancing risk risks thrombotic events thereafter in such difficult requires multidisciplinary approach.
Background We present a 76-year-old poly-morbid male patient suffering from severe ischemic mitral valve regurgitation. A hybrid approach was suggested by the heart team to treat his multifaceted cardiac pathologies. Methods At first percutaneous repair with mitraclip device performed reduce surgical risk. This then followed minimally invasive direct coronary artery bypass (MIDCAB). Results day 20 Post intervention suffered gastrointestinal bleeding which needed argon plasma coagulation...
In this article, we present our initial clinical experience with staged minimally invasive direct coronary bypass (MIDCAB), percutaneous intervention (PCI), and transcatheter aortic valve implantation (TAVI) in high-risk octogenarians (Hybrid).
Erythropoietin (EPO) has been shown to exhibit cardioprotective effects in several studies investigating myocardial ischemia. However, the mechanisms of EPO induced intracardiac cell proliferation and early angiogenesis are poorly elucidated. We hypothesize that may contribute proliferation, eventually induce a beneficial remodeling process. Following permanent LAD ligation rats, (n=99) or saline (n=95) was delivered along infarction border. In control animals without MI (n=55) injected...
Bilateral pneumothoraces are a very rare event. In clinical settings, inadvertent incursion into the pleural space resulting from diagnostic or therapeutic medical interventions such as bilateral venipunctures damage to lung parenchyma due high pressure ventilation may be causative. postcardiac surgery rarely reported. We present case of tension leading up cardiorespiratory arrest in 57-year-old male, weighing 130 kg, who underwent without any complications coronary artery bypass surgery....
