Sources of dietary phosphate differentially contribute to hyperphosphatemia in maintenance haemodialysis (MHD) patients. This cross-sectional study Malaysia investigated association between patterns and serum phosphorus MHD Dietary were derived by principal component analysis, based on 27 food groups shortlisted from 3-day recalls 435 Associations examined with identified patterns. Three emerged: Home foods (HFdp), Sugar-sweetened beverages (SSBdp), Eating out noodles (EO-Ndp). The highest...
Hyperphosphatemia afflicts end-stage chronic kidney disease (CKD) patients, contributing to comorbidities and mortality. Management strategies are dialysis, phosphate binder, limiting dietary intake, but treatment barriers poor patient compliance low health literacy arising from self-efficacy lack of educational resources. This study describes developing validating a mobile application (PMA). The PMA development based on the seven-stage Precaution Adoption Process Model prioritized titrating...
This study aimed to assess the situational capacity for nutrition care delivery in outpatient hemodialysis (HD) setting Malaysia by evaluating dietitian accessibility, practices and patients' outcomes. A 17-item questionnaire was developed administered dialysis managers of 150 HD centers, identified through National Renal Registry. Nutritional outcomes 4362 patients enabled crosscutting comparisons as per accessibility center sector. Dedicated (18%) visiting/shared (14.7%) service...
Kidney biopsy registries all over the world benefit research, teaching and health policy. Comparison, aggregation exchange of data is however greatly dependent on how registration coding kidney diagnoses are performed. This paper gives an overview registries, explores these code disease identifies needs for improvement practice.A literature search was undertaken to identify medical diseases. These were supplemented with information from personal contacts registry websites. A questionnaire...
This study aims to determine the effectiveness of a phosphate mobile app (PMA), MyKidneyDiet-Phosphate Tracker ©2019, on hemodialysis (HD) patients with hyperphosphatemia. A multicenter, open-label, randomized controlled trial design allowed randomization hyperphosphatemia either usual care group (UG; receiving single dietitian-led session an education booklet) or PMA (PG). Thirty-three in each intervention completed 12-week study. Post-intervention, serum phosphorus levels were reduced both...
Personalized medicine in kidney transplantation has the potential to improve outcomes and reduce complications. The aim of this study was investigate influence single nucleotide polymorphisms genes encoding metabolizing enzymes (CYP3A5) transporters (ABCC2) on clinical (acute graft failure and/or acute tubular necrosis (ATN)) transplant recipients (KTR). This a multicenter, retrospective cohort where adult KTR who had undergone between 2020 2021 received tacrolimus-mycophenolate treatment...
Introduction Genetic variations can influence how kidney transplant recipients (KTRs) respond to immunosuppressive drugs. However, limited resources necessitate a cost-benefit analysis of pharmacogenetic testing determine its role in routine practice. This study investigated the cost-effectiveness three genetic polymorphisms (
Introduction: Intravenous calcium gluconate has been used to prevent postoperative hypo-calcaemia (POH) following parathyroidectomy for secondary hyperparathyroidism in chronic kidney disease (CKD). Materials and Methods: Retrospective data were obtained 36 patients with CKD stage 4 5 after parathyroid surgery, correlating albumin-corrected serum the infusion rate of gluconate. Calcium flux was characterised along excursions out target range 2 3 mmol/L. With this data, an improved titration...
Background: Muscle wasting, observed in patients with end-stage kidney disease and protein energy wasting (PEW), is associated increased mortality for those on hemodialysis (HD). Oral nutritional supplementation (ONS) nutrition counseling (NC) are treatment options PEW but research targeting muscle status, as an outcome metric, limited. Aim: We compared the effects of combined (ONS + NC) vs. NC alone status parameters HD PEW. Methods: This multi-center randomized, open label-controlled...
This phase 3, observer-blinded, non-inferiority randomized trial (ClinicalTrials.gov: NCT05517642), conducted from September 2022 to May 2023 at three Malaysian sites, involved 540 adults previously vaccinated with COVID-19 doses. Participants were 1:1 receive either one dose of inhaled Recombinant Vaccine (Ad5-nCoV-IH) or intramuscular tozinameran (BNT-IM). The study assessed safety, vaccine efficacy (VE) and immunogenicity against SARS-CoV-2 variants. primary outcome was the anti-spike...
Abstract Background Hepatitis C virus (HCV) infects more than 71 million people worldwide and chronic HCV infection increases the risk of liver cirrhosis failure. Haemodialysis (HD) is one renal replacement therapies with transmission. Anti-HCV antibodies are serological screening test for that does not detect active phase infection. Majority infected HD patients in Malaysia do have further RNA performed due to high cost thus treatment less frequently offered. Core Antigen (HCV Ag) can...
Protein-energy wasting (PEW) is a devastating metabolic derangement that leads to increased morbidity and mortality in hemodialysis (HD) patients. This study aimed determine the diagnostic test accuracy of bioelectrical impedance analysis derived-phase angle (PhA) detecting PEW among HD was multi-centre, cross-sectional conducted amongst 152 multi-ethnic patients Klang Valley, Malaysia. assessed using International Society Renal Nutrition Metabolism criteria as reference method. PhA measured...
<h3>ABSTRACT</h3> Carbamazepine remains a first-line antiepileptic medication for the treatment of partial seizures. Despite its widespread use, carbamazepine has significant neurotoxicity and hypersensitivity reactions. We report case patient post-kidney transplant who was on regular childhood epilepsy developed nystagmus, diplopia broad-base gait after receiving diltiazem. Understanding interaction between diltiazem is necessary to prevent neurotoxic effects.

<h3>Background and Aims</h3> In lupus nephritis (LN), complete remission (CR) or partial is associated with better patient renal survival. Subjects who do not achieve a 25% reduction in proteinuria within 8 weeks of starting induction immunosuppression are unlikely to even PR. Voclosporin (VCS) novel CNI demonstrating less pharmacokinetic–pharmacodynamic variability potentially improved safety profile compared other CNIs. <h3>Methods</h3> Entry criteria biopsy 24 months (Class III; IV-S,...
<h3>Background</h3> In lupus nephritis (LN), complete or partial remission (PR) is associated with better patient and renal survival. However, studies demonstrate that subjects who did not achieve a 25% reduction in proteinuria within 8 weeks of starting induction immunosuppression were unlikely to even PR. Recent suggest combination calcineurin inhibitors (CNi9s), MMF low dose steroids may be effective LN patients. Voclosporin (VCS) next generation CNi intended for the treatment autoimmune...
Abstract The metabolic impact of circulating fatty acids (FAs) in patients requiring hemodialysis (HD) is unknown. We investigated the associations between plasma triglyceride (TG) FAs and markers inflammation, insulin resistance, nutritional status body composition. Plasma TG-FAs were measured using gas chromatography 341 on HD (age = 55.2 ± 14.0 years 54.3% males). Cross-sectional with 13 examined multivariate linear regression adjusted for potential confounders. Higher levels TG saturated...
