上行大動脈の偽腔が開存していた(上行大動脈偽腔開存型の)逆行性A型急性大動脈解離に対して,胸部下行大動脈のentryを閉鎖する目的で,発症当日に緊急胸部大動脈ステントグラフト内挿術を行い,術後の造影CT検査で上行から近位胸部下行大動脈の偽腔が,術後に完全に血栓化し縮小(真腔が拡大)して消失した,49歳・男性の症例を経験したので報告する.上行大動脈偽腔開存型の逆行性A型急性大動脈解離に対する胸部大動脈ステントグラフト内挿術は,体外循環・心停止・低体温循環停止などを伴う大動脈置換に比べはるかに低侵襲であり,entryが左鎖骨下動脈分岐部から少なくとも2 cm程度以上は末梢に位置するような,限られた症例には有用な治療になる可能性がある.
