インスリンは,血糖値の上昇に応じて血中へと分泌され,糖の取り込みと糖代謝を促す重要なホルモンで膵臓ランゲルハンス島のβ細胞で合成される.インスリンの分泌や合成が低下すると,高血糖が持続する糖尿病を発症することが知られている.したがって,膵臓β細胞で,インスリン合成がどのように行われ,生理的にどのように正確に制御されているのかを理解することは,インスリン合成の観点からだけでなく,糖尿病の予防および治療の観点からも非常に重要である.インスリン前駆体のプロインスリンは,小胞体と呼ばれる細胞小器官で合成され,3つのジスルフィド結合形成を介して酸化的に正しく折り畳まれる.これまで,プロインスリンの折り畳み酵素やそれらの酵素群の発現制御の機構は未解明のままであった.今回,私たちは遺伝子改変マウスと膵臓由来β培養細胞の解析により,小胞体ストレスセンサーIRE1αが膵臓β細胞で5つの酵素遺伝子を転写誘導することで,プロインスリンの折り畳みを促進していることを見いだした(1).

直腸pm癌の原発巣の病理組織学的所見がどの程度予後に反映されているかを検討した.1975年から1988年の間に当院で手術を行った直腸pm癌62例を対象とした.転移再発した14例を予後不良群-A群-, 転移再発を認めない48例を予後良好群-B群-として2群を比較した.A群では高分化型腺癌3例, 中分化型腺癌11例であり, B群ではそれぞれ29例, 19例であった.固有筋層への浸潤程度pm1, pm2, pm3に分けるとA群ではそれぞれ2例, 4例, 8例, B群ではそれぞれ18例, 19例, 11例であった.静脈侵襲陽性率はA群で71.4%, B群で35.4%, リンパ節転移陽性率はA群で64.3%, B群で12.5%であった.これらについては2群間に有意差を認めたが, INFとリンパ管侵襲は, 2群間に有意差を認めなかった.以上の結果より, 直腸pm癌においては癌の分化度, 固有筋層への浸潤程度, 静脈侵襲, リンパ節転移が予後を推測する上で重要な因子であることが示唆される.

線毛運動異常症 (ICS) の20歳女性と嚢胞性線維症 (CF) の11歳男子における肺緑膿菌感染 増悪の治療経験を比較検討した。ICS例は息切れ・発熱を主訴とし, 著明な発熱・炎症反応亢進・胸部レントゲンにおける肺野の浸潤影を特徴とした。これに対して, CF例は息切れ・排疾困難を主訴 とし, 発熱を伴わず呼吸困難が強かった。炎症反応は中等度陽性であり, 胸部レントゲンでは肺門 付近の陰影を主体としていた。気管支拡張症における肺緑膿菌感染増悪の形態は, 肺炎を主体とする型と, 喀疾による気道閉塞 を主体とする型があると考えられ, 肺炎型では抗生剤・ガンマグロブリン療法等が治療の中心であ るが, 気道閉塞型では抗生剤投与以外に理学療法・吸入療法による排疾促進が重要であった。
